白内障术后视网膜脱落临床特征及疗效分析

Clinical Characteristics and Curative Effect Analysis of Retinal Detachment after Cataract Operation

目的:研究分析白内障术后视网膜脱落患者的临床特征,以及分析不同白内障手术患者的术后康复情况与视力恢复影响因素。方法:选择2015年8月~2016年8月于某院就诊的白内障术后视网膜脱落患者84例,84例患者均为单眼进行白内障手术后出现视网膜脱落的现象。对84例患者的病历进行研究分析,明确84例患者在白内障手术后出现视网膜脱落的临床特征,以及患者的基本情况与患病具体情况等等。结果:根据研究结果显示,84例患者视网膜脱落类型主要为孔源性视网膜脱落,并且孔源性视网膜脱落时主要伴有黄斑裂孔、合并脉络膜脱落两种状况。其次,84例患者在手术前大部分为查出裂孔,所有患者的视网膜脱落范围数3个象限为主,其次为1个象限、2个象限、4个象限。而患者的裂孔数以1个为主,其次为3个以上、2个、0个。另外,84例患者中,75例患者进行视网膜脱落手术的一次复位成功,一次复位成功率为89.29%;84例患者手术前的整体平均视力为(2.85±1.14),手术后的整体平均视力为(3.64±0.90),所有患者手术前后的整体平均视力以及视力变化组间数据对比差异性显著(P<0.05)。结论:白内障术后视网膜脱落患者的视网膜脱落类型主要为孔源性视网膜脱落,一般伴有黄斑裂孔、合并脉络膜脱落两种状况,患者手术前则难以查出裂孔;并且白内障术后视网膜脱落患者进行且白内障术后视网膜脱落手术的一次复位率高,患者在手术后的视力变化十分显著。

Objective：To research and analyze the clinical characteristics of amotio retinae after cataract operation,postoperative rehabilitation of patients with different cataract surgery,and factors affecting the visual acuity.Methods：84cases of retinal detachment after cataract surgery treated in a hospital from August2015 to August 2016 were selected.All patients received the monocular cataract surgery and appeared the phenomenon of retinal detachment.Studied and analyzed the medical records of 84 patients,cleared the clinical features of retinal detachment after cataract surgery,and the basic situation and specific conditions of disease of 84 patients.Results：Based on the research results,the main type of retinal detachment of 84 cases was rhegmatogenous retinal detachment,which was mainly accompanied with macular hole and choroidal shedding.Secondly,most of the 84 patients were found out holes before surgery,and the scope of retinal detachment of all patients were main 3quadrants,followed by 1quadrant,2quadrants and 4quadrants.The number of holes of patients was main 1,followed by more than 3,2,and 0.In addition,75 cases of patients restored successful through retinal detachment surgery,the success rate was 89.29%;The overall average visual acuity of 84 patients before surgery was（2.85±1.14）,after the surgery,the overall average visual acuity was（3.64±0.90）,and the difference of overall average visual acuity change of all patients and visual acuity change between the groups before and after surgery were statistically significant（P〈0.05）.Conclusion：The main type of retinal detachment after cataract surgery was rhegmatogenous retinal detachment,which is generally associated with macular hole and choroidal shedding.Hiatus are very difficult to identify for patients before surgery.After cataract surgery,the reset rate of patients treated with retinal detachment surgery at a time is high,and patients＇ post-operative visual acuity change is very remarkable.
