摘要
目的:探讨可溶性髓样细胞触发受体-1(s TREM-1)表达和肝硬化自发性细菌性腹膜炎(SBP)患者肠黏膜通透性的关系。方法:将23例肝硬化腹水并发SBP患者作为SBP组,33例肝硬化腹水患者作为非SBP组,20例健康体检者作为健康对照组,检测血清和腹水s TREM-1的表达水平,并计算尿乳果糖排泄率与甘露醇排泄率比值(LAC/MAN),探讨s TREM-1及LAC/MAN在上述患者(人群)中有无差别及其相关性。结果:SBP组血清及腹水s TREM-1的表达分别为(217.28±87.65)pg/m L和(218.76±69.38)pg/m L,均高于非SBP组(55.51±14.28)pg/m L和(75.98±21.57)pg/m L(P<0.05);SBP组血清及腹水s TREM-1值与LAC/MAN比值呈正相关(r血清=0.518,r腹水=0.527,P<0.05),血清及腹水s TREM-1值与Child-Pugh评分呈正相关(r血清=0.791,r腹水=851,P<0.05)。结论:s TREM-1可增加自发性腹膜炎患者的肠黏膜通透性,而且与肠黏膜通透性损伤程度及肝功能损害相关。
ObjectiveTo investigate association of soluble triggering receptors expressed by myeloid cell-1(s TREM-1) on intestinal mucosal permeability in cirrhotic patients with spontaneous bacterial peritonitis(SBP).MethodsTwenty-three patients with SBP were taken as SBP group, the concurrent 33 cirrhotic ascites pa-tients without SBP were taken as non-SBP group, and 20 healthy cases as healthy control group. The serum andascitic fluid levels of s TREM-1 were measured, the levels of urinary excretion of LAC/MAN ratios were calculat-ed.ResultsThe s TREM-1 concentration of(217.28± 87.65) pg/m L in serum and(218.76±69.38) pg/m L in as-cites in SBP group were both higher than those in non-SBP group [(55.51±14.28) pg/m L,(75.98±21.57) pg/m L,respectively] with statistical differences between the two groups(P〈0.05). The levels of the serum and asciticfluid s TREM-1 had significantly correlated with the levels of urinary excretion of LAC/MAN ratios(rserum=0.518,rascites=0.527(P〈0.05),and the levels of the serum and ascitic fluid s TREM-1 had significantly correlated withChild-Pugh classification(rserum =0.791,rascites=851).Conclusions TREM-1 may promote intestinal mucosalpermeability in patients with SBP, and is also associated with the degree of intestinal permeability damage andimpaired liver function.
