痔病的中医辨证论治及中药用药规律

The Syndrome Differentiation Treatment and Traditional Chinese Medicine Laws of Hemorrhoids Disease

目的:通过对广州中医药大学附属广州市中医医院肛肠科的门诊病历进行数据收集整理,分析痔病的中医辨证论治及中药用药规律,以指导临床实践。方法:收集符合研究标准的门诊病历,录入有效信息,建立数据库。用Access进行数据整理,SPSS软件进行统计分析。结果:在符合标准的257名患者中,最常见的痔病中医证型为脾虚气陷证,为146例(56.81%)。其次为湿热下注证,为49例(19.07%)。使用频次最高的中药依次为枳壳、槐花、白术,其性多寒、温。味多甘、苦。药物归经,以脾、胃两经最为多见。多为补虚、清热、收敛、活血化瘀药。结论:痔病治疗的中药用药规律,应符合"未病先防,既病防变"的中医治病理念,体现"肛肠疾病从气血论治"的学术思想。

ObjectiveTo Collect the data of outpatient medical records and analyse, the syndrome differen-tiation treatment and Traditional Chinese Medicine laws of hemorrhoids disease, so to guide clinical practice.MethodsThe outpatient service medical records which meted the diagnostic were entered valid information,and set up the database. All data were collected with Microsoft Access and analyzed by SPSS software.Results There were 146 cases（56.81%） among 257 patients belonged to syndrome of Sinking of Qi due to Spleen Defi-ciency accroding to TCM Syndrome Types of hemorrhoidal disease. The cases of Syndrome of Dampness-heatPouring Downward were 49 cases（19.07%）. Most frequently used drug were Fructus aurantii, Flos Sophorae andAtractylodes, whose property of cold and warm. The property and flavor of these herbs were predominantly sweetand bitter. Drug tropism was mostly entered to spleen and stomach meridian. These herbs were tonify deficiency,heat-clearing, restrain self＇s medicine, promoting blood circulation by removing blood stasis.ConclusionThelaws of Traditional Chinese Medicine in treating hemorrhoids disease should comply with the＂prevention first,treatment second concept. The academic thoughts of treated anorectic diseases from QI and blood should be re-flected.
