袖珍负压装置的设计及在脂肪液化伴感染坏死伤口中的应用

Effect of a pocket-type negative-pressure wound closure device in the treatment for fat liquefaction with infection in wounds

目的观察自行设计的袖珍负压装置在脂肪液化伴感染坏死伤口中的应用效果。方法选择56例冠状动脉搭桥术后取大隐静脉处脂肪液化伴伤口感染坏死的患者,随机分为实验组30例和对照组26例。实验组采用自行设计的袖珍负压装置换药治疗,对照组采用常规负压封闭换药治疗,比较两组的治疗效果。结果两组伤口进入红期时间、伤口面积愈合率、伤口愈合时间、换药次数比较,差异均无统计学意义(t值分别为0.695,0.531,-0.075,-0.582;P>0.05);两组总费用比较,差异有统计学意义(t=-62.766;P<0.001)。结论袖珍负压装置能促进脂肪液化伴感染坏死伤口愈合,提高伤口愈合的质量,较常规负压封闭治疗具有更经济、更实用、更便携的优点。

Objective To evaluate the effect of a self-designed pocket-type negative-pressure wound closure de- vice in the treatment for fat liquefaction with infectious necrosis in wounds. Methods Fifty-six patients who devel- oped fat liquefaction with infection in the great saphenous vein after coronary artery bypass grafting were enrolled and divided into two groups using random number table. Patients in the treatment group were treated with pockettype negative pressure closure device,while patients in the control group were routinely treated with VAC. Therapeutic effects in the two groups were compared. Results There were no statistically significant differences in the time for wound to enter into the red period,the wound-area healing rate,the wound healing time and dressing change frequency between two groups（t=0.695,0.531,-0.075,-0.582;P〉0.05）; the total cost was lower in the treatment group than that in the control group,and the difference was statistically significant（t=-62.766;P〈0.001）. Conclusion The pocket-type negative-pressure wound closure device can promote the healing of wounds with fat liquefaction and infectious necrosis,and improve the quality of wound healing. Compared with CVD,the device has the advantages of simple operation ,low cost,portability and high effectiveness ,which is worthy of popularization.
