自制椎间孔扩大成形装置在经皮腰椎间孔镜手术中的应用 被引量:5

摘要 目的探讨自制椎间孔扩大成形装置在经皮椎间孔镜下髓核摘除术中应用的安全性和临床价值。方法选择2014年10月至2015年6月收治的60例单纯腰椎间盘突出症患者,男34例,女26例;年龄22~81岁,平均51岁。60例患者均采用经皮椎间孔镜手术方式治疗,术中利用自制椎间孔扩大成形装置行椎间孔扩大成形。结果 60例患者顺利完成手术,无须利用镜下动力系统再次磨骨。手术操作时间(61.0±13.7)min,术中X线透视次数(11.5±2.8)次。所有患者随访时间6~14个月,平均8.5个月。随访结果显示,患者术前视觉模拟评分(visual analogue scale,VAS)为(7.6±1.1)分,术后1个月腿痛VAS评分为(2.4±0.7)分,术后6个月腿痛VAS评分为(2.1±0.8)分,经配对t检验,手术前后比较,差异有统计学意义(P<0.01)。最后随访时根据改良的MacNab标准,优33例(55%),良23例(38.3%),可2例(3.3%),差2例(3.3%),优良率为93.3%。结论利用自制椎间孔扩大成形装置在经皮椎间孔镜下腰椎间盘突出症髓核摘除术中行椎间孔扩大成形,方法简单,安全可靠,能减少医患人员放射线接触剂量。
