复方甘草酸苷片治疗早产儿胃肠外营养相关性胆汁淤积的临床疗效观察

Clinical Study on the Therapeutic Effect of the Compound Glycyrrhizin Tablet on Parenteral Nutrition Associated Cholestasis in Preterm Newborn Infants

目的:观察复方甘草酸苷治疗早产儿胃肠外营养相关性胆汁淤积(PNCA)的疗效及不良反应。方法:62例PNCA早产儿按随机数字表法分为治疗组与对照组,每组各31例。均给予相同的基础支持治疗,治疗组在此基础上给予复方甘草酸苷片12.5 mg/次,口服(冲服),2次/d,2周为1个疗程,连续治疗2个疗程。比较两组患儿治疗2、4周后血清总胆红素(TBIL)、直接胆红素(DBIL)、丙氨酸氨基转移酶(ALT)与总胆汁酸(TBA)水平,并计算总有效率、观察不良反应的发生情况。结果:复方甘草酸苷治疗后2、4周后TBIL、DBIL、ALT和TBA水平与对照组比较差异均有统计学意义(P<0.05或P<0.01);两组总有效率分别为93.5%、83.9%,比较差异均有统计学意义(P<0.05);两组均未出现严重不良反应。结论:复方甘草酸苷治疗早产儿PNCA疗效较好,且无严重不良反应。

Objective： To observe the clinical effects and the adverse reactions of the compound glycyrrhizin tablet on parenteral nutrition associated cholestasis （ PNCA ） in preterm newborn infants.Method ： 62 preterm newborn infants with PNCA were divided into control group and treatment group according to random number table method, 31 cases in each group.They were given the same basic treatment, the treatment group was administered the compound glycyrrhizin tablet orally 12.5 mg/time, 2 times a day, a course of treatment was 2 weeks for 2 courses.The levels of total bilirubin （ TBIL ）, direct bilirubin （ DBIL ）, alanine aminotransferase （ ALT ） and total bile acid （ TBA ） in serum in the two groups after 2 and 4 weeks were compared.The total effective rate and the adverse reactions were observed.Result： Compared with the control group, the levels of serum TBIL, DBIL, ALT and TBA in the treatment group were lower than control group （ P〈0.05 or P〈0.01 ） . The total effective rate in the treatment group was 93.5%, and the control group was 83.9%, the difference was statistically significant （P〈0.05） .There was no significant adverse reactions in the two groups （ P〉0.05 ） . Conclusion： The compound glycyrrhizin showed a good therapeutic effect on the PNCA in preterm newborn infants.There was no significant adverse reactions.
