经尿道双极等离子前列腺剜除术对良性前列腺增生尿道功能与性功能的影响

Effect of Transurethral Plasmakinetic Prostatectomy on Urethral Function and Sexual Function in Benign Prostatic Hyperplasia

目的:研究分析经尿道双极等离子前列腺剜除术对良性前列腺增生尿道功能与性功能的影响。方法:选取2014年7月-2016年6月收治的良性前列腺增生的患者78例,按治疗方式的不同分为观察组和对照组,每组各39例。对照组患者采用经尿道双极等离子前列腺电切术治疗,观察组患者采用经尿道双极等离子前列腺剜出术。比较两组患者治疗前后IPSS、QOL、MFR评分以及手术的时间、术中出血量、膀胱冲洗时间、导尿管置留的时间,并对患者术后进行3个月跟踪随访,采用男性性功能自我评价表对术后性功能情况进行比较。结果:治疗后,两组患者的IPSS、QOL、MFR评分较治疗前均有显著的改善,比较差异有统计学意义(P<0.05);观察组性功能重度发生率为7.69%,对照组为17.95%,两组比较差异有统计学意义(P<0.05)。对照组手术时间、术中出血量、膀胱冲洗时间和导尿管置留时间均高于观察组,两组比较差异均有统计学意义(P<0.05)。结论:经尿道双极等离子前列腺剜除术对良性前列腺增生尿道功能与性功能改善效果显著,安全可靠,值得临床推广。

Objective： To study the effects of transurethral plasmakinetic enucleation of prostate on urethral function and sexual function in patients with benign prostatic hyperplasia.nethod： 78 patients with benign prostatic hyperplasia （ BPH ） in our hospital were selected from July 2014 to June 2016 as objects, they were divided into observation group and control group according to different treatment ways, 39 cases in each group.The patients in the control group were treated with transurethral bipolar plasmakinetic prostatectomy.The patients in the observation group were treated with transurethral bipolar plasmakinoplasty.The scores of IPSS, QOL and MFR, operation time, blood loss amount, bladder irrigation time and catheter retention time of two groupswere compared.The patients were followed up 3 months after operation.The Male sexual function situation after surgery was compared by self-assessment scale.Result： After treatment, the IPSS, QOL and MFR scores of two groups were significantly improved compared with before treatment, the differences were statistically significant （P〈0.05） ; the incidence of severe sexual function in the observation group was 7.69%, the control group was 17.95%, the difference was statistic significant （ P〈0.05 ） .The operation time, intraoperative blood loss, bladder irrigation time and catheter retention time were significantly higher in the control group than those of the observation group, the differences were statistically significant （ P〈0.05 ） .Conclusion： The effects of transurethral bipolar plasmakinetic prostatectomy for benign prostatic hyperplasia urethral function and sexual function improvement are significant, it is safe and reliable, worthy of clinical promotion.
