化痰祛瘀浓煎剂治疗瘰疬痰瘀互结证60例临床研究

Clinical Study on 60 Cases of Surface Lymphatic Tuberculosis Patients with Syndrome of Phlegm and Blood Stasis by Self-Made Preparations of "Huatan Quyu"

目的:观察化痰祛瘀浓煎剂治疗瘰疬痰瘀互结证的疗效,探讨化痰散结活血祛瘀法治疗本病的作用机制。方法:将60例瘰疬痰瘀互结证患者随机分为对照组与治疗组各30例,均采用常规抗结核化疗药物(3RHE/6RH)治疗;治疗组予化痰祛瘀浓煎剂治疗,对照组予消瘰丸治疗。治疗前、治疗后(第7、14、28天)分别进行实验室检查,并观察记录中医证候评分。结果:两组中医证候疗效比较,治疗组优于对照组,差异有统计学意义(P<0.05)。两组治疗后中医证候评分较治疗前降低,差异均有统计学意义(P<0.05)。治疗组治疗后中医证候评分低于对照组,差异有统计学意义(P<0.05)。治疗后两组比较,治疗组凝血酶、纤维蛋白原、脂蛋白、血沉及LDL检测值均低于对照组,HDL高于对照组;治疗后治疗组IFN-γ水平显著高于对照组,差异均有统计学意义(P<0.05)。结论:化痰祛瘀浓煎剂治疗瘰疬痰瘀互结证有较好疗效。

Objective： To observe the curative effect of self-made ＂Huatan Quyu ＂ concentrated decoction for surface lymphatic tuberculosis, in order to explore the possible mechanism about the method of dissipating phlegm and resolving masses treating surface lymphatic tuberculosis from change of clinical and laboratory data. Methods： The 60 patients with body surface of lymphoid tuberculosis were randomly divided into control and treatment groups, 30 cases in each; patients were given routine anti-tuberculosis chemotherapy drugs （SRHE/ 6RH） in two groups. Patients were treated with ＂Huatan Quyu＂concentrated decoction in treatment group and Xiaoluo pill （消瘰丸） in control group. Before and after the treatment （7th, 14th, 284 day）, laboratory examina- tion, and TCM syndrome scores data were gathered for statistical analysis. Results： The treatment group showed better TCM syndrome effect than the control group （P〈0.05）. The TCM syndrome scores decreased in the two groups after treatment, with statistical difference （P〈0.05）. And the treatment group showed lower TCM syndrome scores than the control group after treatment, with statistical difference （P〈0.05）. After the treatment, the treat- ment group showed lower Thrombin, fibrinogen, lipoprotein, ESR and LDL than the control group as well as higher HDL and IFN-γ level, with statistical difference （P〈0.05）. Conclusion： The self-made ＂Huatan Quyu ＂ concentrated decoction shows good curative effect on surface lymphatic tuberculosis.
