真武汤合当归芍药散治疗原发性肾病综合征的疗效观察

Primary Nephritic Syndrome Patients Treated with Combination of Zhenwu Decoction and Danggui Shaoyao Decoction

目的:观察真武汤合当归芍药散治疗原发性肾病综合征的临床疗效,探索治疗原发性肾病综合征的新方法。方法:选取2008年2月—2015年8月期间就诊原发性肾病综合征患者84例,随机分为(中药口服+西医治疗)、对照组(西医治疗)各42例,对比两组患者治疗前后的总体疗效、中医症候积分及疗效、24 h尿蛋白定量、血浆白蛋白、血脂、肾功能以及血清IL-8和IL^(-1)3的表达。结果:(1)两组治疗后的总体疗效:实验组的总有效率为88.1%,对照组为71.4%,实验组明显优于对照组(P<0.05);(2)中医症候积分及疗效:治疗后,实验组和对照组均较治疗前有明显的改善(P<0.05),而实验组明显优于对照组(P<0.05);实验组的总有效率明显优于对照组(P<0.05);(3)理化指标方面:实验组和对照组在24 h尿蛋白定量、血脂、肾功能、血浆白蛋白、血清IL-8和IL^(-1)3等方面均有明显的改善作用(P<0.05),而实验组明显优于对照组(P<0.05)。结论:真武汤合当归芍药散可以明显提高原发性肾病综合征患者的总体疗效,改善临床症状,降低24 h蛋白尿、血脂、肾功能,升高血浆白蛋白,调节炎症因子IL-8、IL^(-1)3的表达,减少不良反应,提高患者的生存质量。

Objective：To observe the clinical effect of the treatment of primary nephritic syndrome patients with the combination of Zhenwu Decoction and Danggui Shaoyao Decoction, and explore the new method of treating primary nephritic syndrome. Methods：From February 2008 to August 2015, eighty -four patients with primary nephritic syndrome were randomly divided into treatment group and control group, comparing two groups＇ TCM symptoms integral and curative effect ,24 hours urinary protein, plasma albumin ,blood lipid ,renal function and serum IL- 8 and IL- 13 expressions before and after treatment. Results ： （ 1 ） The overall efficacy of the two groups after treatment ： the total effective rate of the experimental group was 71.4% and the control group＇ s was 88.1%. The experimental group was significantly better than the control group （P 〈 0.05）. （2） TCM syndrome integral and curative effect：after treatment, the experimental group and the control group＇ s were significantly improved （P 〈 0.05 ） while the experimental group was significantly better than the control group （ P 〈 0.05 ）. The total effective rate of the experimental group was significantly better than the control group （ P 〈 0.05 ）. （3） Physical and chemical indicators ： the experimental group and the control group＇ s 24 hours urine protein ,blood lipids ,renal function, serum albumin, serum IL - 13 and IL - 8 were significantly improved （ P 〈 0.05 ） while the experimental group was significantly better than the control group （ P 〈 0.05 ）. Conclusion ： Zhenwu Decoction and Danggui Shaoyao Decoction can significantly improve the overall efficacy of patients with primary nephritic syndrome and the clinical symptoms, reduce 24h urine protein, serum lipid, renal function and serum albumin, regulate the expressions of inflammatory cytokines IL -8 and IL - 13, reduce the adverse reactionsand improve the quality of patients living.
