保洁员手卫生依从性培训对高原肺心病住院患者肺部感染的影响分析

Effect of hand hygiene compliance training on pulmonary infection in patients with high altitude pulmonary heart disease

目的探讨保洁员手卫生依从性培训对高原肺心病住院患者肺部感染的影响,为降低肺部感染发生率提供参考依据。方法对2015年12月-2016年5月医院上岗11名保洁员进行手卫生依从性培训,比较培训前后共37例患者肺部感染发生率、保洁员手部细菌总数及手卫生依从性。结果培训前,受检患者的肺部感染率为20.00%,培训后,受检患者的肺部感染率为5.88%;培训前,11名保洁员手部细菌总数均超过10CFU/cm^2,其中11~100CFU/cm^2 3例,101~500CFU/cm^2 6例,>500CFU/cm^2 2例;培训后,11名保洁员的手部细菌总数均不超过500CFU/cm^2,其中≤10CFU/cm^2 7例,11~100CFU/cm^2 2例,101~500CFU/cm^2 2例。培训前,保洁员操作前、操作后、接触患者前、接触患者后的手卫生依从性分别为54.55%、36.36%、63.64%、63.64%,培训后分别为81.82%、72.73%、72.73%、90.91%。结论开展保洁员手卫生依从性培训能够降低高原肺心病住院患者肺部感染的发生风险,提高保洁员手卫生依从性,值得临床推广。

OBJECTIVE To investigate the effect of hand hygiene compliance training on pulmonary infection in patients with high altitude pulmonary heart disease,so as to provide references for reducing the incidence of pulmonary infection.METHODS Totally 11 cleaning staff were given hand hygiene compliance training from Dec.2015 to May 2016.The 37 cases of pulmonary infection rate,the total number of bacteria and hand hygiene compliance before and after the training were compared.RESULTS Before training,the pulmonary infection rate of patients was 20.00%,and after the pulmonary infection rate was 5.88%.Before training,the total bacteria count in hand of 11 cleaning staff were more than 10CFU/cm^2,including three cases of 11-100CFU/cm^2,six cases of 101-500CFU/cm^2,and 2cases of 500CFU/cm^2;and after training,the total bacteria count did not exceed 500CFU/cm^2,of which 7cases were≤10CFU/cm^2,two cases were 11-100CFU/cm^2,and two cases were 101-500CFU/cm^2.Before training,the hand hygiene compliance of cleaning staff before operation,after operation,before contacting with the patients,and after contacting with the patients were 54.55%,36.36%,63.64%,and 63.64%,respective.After the training,the hand hygiene compliance were 81.82%,72.73%,72.73%,and90.91%,respectively.CONCLUSIONTo carry out cleaning staff hand hygiene compliance training can reduce the risk of pulmonary infection in patients with high altitude pulmonary heart disease,and improve the hand hygiene compliance of the cleaning staff,so it is worthy of clinical promotion.
