MR-DWI及ADC值对乳腺癌诊断价值的研究

目的:探讨磁共振扩散加权成像(MR-DWI)及其表观弥散系数(ADC)值对乳腺癌的诊断价值。方法:选择2013年10月-2015年12月本院手术治疗的87例乳腺癌患者为研究组,同期87例乳腺炎患者为对照组,术前行双侧乳腺MRI检查,测量肿块ADC值,术后组织病理学检查分级,并分析其相关性。结果:研究组MR-DWI全部可见不均匀高信号,部分可见显著高信号灶,而对照组可见等信号和低信号,两组比较差异有统计学意义(P<0.05)。所有患者乳腺癌病灶ADC值显著低于对照组,两组比较差异有统计学意义(P<0.05)。乳腺癌Ⅰ、Ⅱ和Ⅲ级病灶组织的ADC值逐渐降低,其ADC均值分别为(1.106±0.124),(0.607±0.056),(0.211±0.039),各组间比较差异有统计学意义(P<0.05)。不同乳腺癌类型间ADC值比较,差异无统计学意义(P>0.05)。ADC值与乳腺癌病理组织分级呈负相关(r=-0.947,P<0.05)。结论:MR-DWI及ADC值可诊断乳腺癌,ADC值与乳腺癌病理组织分级负相关,ADC值越小乳腺癌病理组织分级越严重。

Objective：To study the diagnosis value of magnetic resonance diffusion-weighted imaging（MR-DWI） and apparent diffusion coefficient（ADC） value in breast cancer. Method： 87 patients with breast cancer cured by operative treatment were selected as research group in our hospital from October 2013 to December 2015, and 87 patients with mammitis were selected as control group.The patients were exanimated bilateral breast MRI and the ADC values were measured by preoperative, postoperative histopathological examination and relevance were analyzed.Result：The patients in research group of breast lesion MR- WI were all visible heterogeneous high signal, partly visible focal high signal, and the control group was visible other signal and low signal,the difference was statistically significant（P〈0.05）.The ADC values of all patients in research group of lesions significantly were lower than that of control group, the differences were statistically significant（P〈0.05）.The ADC values in research group of Ⅰ,Ⅱ and Ⅲ lesions were reduced, their average ADC were respectively（1.106±0.124）,（0.607±0.056） and（0.211±0.039）,the differences were statistically significant（P〈0.05）.ADC value between different types of breast cancer was no statistically significant difference（P〉0.05）.The ADC values were negatively correlated with breast cancer histopathological grading（r=-0.947,P〈0.05）.Conclusion：The MR-DWI and ADC values can diagnosis of breast cancer,ADC values is negatively related to the breast cancer histopathological grading,the smaller of ADC values,the more serious of histopathologic classification of breast cancer.
