针刺治疗阿片类药物相关便秘临床观察

Effect of acupuncture on opioid-related constipation

目的:观察针刺治疗对阿片类药物相关便秘的临床疗效,并与口服药物的疗效进行比较。方法:前瞻性随机对照研究。60例符合纳入标准的病例,采用数字表法随机分为2组,治疗组(行针刺和口服乳果糖口服液治疗)、对照组(口服乳果糖口服液)各30例。观察2组治疗前、治疗1周、治疗2周、停止治疗1周后的便秘的改善及生活质量的提高情况,采用主观症状半定量计分法评分,对两组的评分进行对比分析。结果:治疗组与对照组比较,在腹胀、每次排便时间和便质改善上在各时点均有显著性差异(均P<0.05)。对照组在便意感、排便间隔时间、排便费力程度、排便不尽感改善上优于治疗组(均P<0.05)。治疗组食量、疲乏、睡眠的改善在各时点比较差异均有统计学意义(均P<0.05)。结论:针刺治疗能够明显改善阿片类药物相关便秘患者的临床症状,提高生活质量。

Objective：To observe the effect of the acupuncture treatment on the constipation induced by taking opioid.Methods：A total of 60 patients with opioid-induced constipation were randomly divided into acupuncture group（treated with lactulose oral solution plus acupuncture,n=30）and control group（treated with lactulose oral solution only,n =30）,the therapeutic effect was compared after 1-,2-week duration of treatment and 1week after stopping treatment.Results：The acupuncture plus lactulose had a better effect on the abdominal distension,duration of defecation and the texture of the stool than lactulose only（all P〈0.05）.While the lactulose group was better effect on awareness of defecation,defecating time,difficult defecation and defecation feeling of heaviness than acupuncture plus lactulose group（all P〈0.05）.The acupuncture group had significant differences on the approvement of appetite,fatigue and sleep at following time points：1-,2-week treatment,and 1week after cessation of the treatment（all P〈0.05）.Conclusion：Acupuncture treatment can effectively alleviate constipation due to administration of oral opioids and improve the quality of life.
