分析老年高血压患者麻醉诱导期的心血管功能变化

Analysis of the Changes of Cardiovascular Function in Elderly Hypertensive Patients During Induction of Anesthesia

目的对老年高血压患者麻醉诱导期的心血管功能变化情况进行分析。方法将本院在2016年1-10月所收治的需行手术治疗的老年高血压患者140例作为研究对象,使用丙泊酚、芬太尼等进行麻醉诱导,并给予气管插管。比较本组患者麻醉诱导期间期平均动脉压、心率以及心率变异性的差异。结果在麻醉诱导前,本组患者心率为(78.25±12.05)次/min,插管即刻为(86.57±7.58)次/min,插管后3 min为(82.25±5.25)次/min,平均动脉压在诱导前为(95.68±10.25)Mp/mm Hg,插管前为(73.68±4.85)Mp/mm Hg,插管即刻为(86.74±12.68)Mp/mm Hg,插管后3 min为(75.68±8.57)Mp/mm Hg。在心率变异性HRV上,麻醉诱导后,本组患者高频功率(HF)与低频功率(LF)均存在一定程度下降,而低频/高频则无明显差异,麻醉诱导前为(0.65±0.19),插管后为(0.64±0.22),P>0.05,差异无统计学意义。结论在诱导治疗期间,老年高血压患者心血管功能存在较为明显变化,为防止患者出现低血压的症状,需适当对血容量进行补充。

Objective The changes of cardiovascular function in elderly hypertensive patients during anesthesia induction period. Methods 140 cases of elderly patients with hypertension who were treated in our hospital from January to October 2016 were treated with propofol and fentanyl, and were given endotracheal intubation. The mean arterial pressure, heart rate and heart rate variability during anesthesia induction were compared between the two groups. Results Before the induction of anesthesia, heart rate in this group of patients was（78.25±12.05） /min, intubation for（86.57±7.58） /min, 3 min after intubation for（82.25±5.25） /min, the mean arterial pressure before induction for（95.68±10.25） Mp/mm Hg, before intubation（73.68±4.85） Mp/ mm Hg, immediately after intubation for（86.74±12.68） Mp/mm Hg, 3 minutes after intubation for（75.68±8.57） Mp/mm Hg. Heart rate variability in HRV, after induction of anesthesia, the patients of high frequency power（HF） and low frequency power（LF） has a certain degree of decline, while the low frequency / high frequency has no obvious difference before induction of anesthesia for（0.65±0.19）, after intubation for（0.64±0.22）, P〉0.05, difference no statistically significant. Conclusion During induction therapy, cardiovascular function in elderly patients with hypertension has obvious changes, in order to prevent the symptoms of patients with hypotension, appropriate for blood volumesupplement.
