摘要 目的探讨肺炎支原体(MP)感染对患儿细胞免疫、肺功能的影响及其相关性。方法选取2013年11月—2014年1月于苏州大学附属儿童医院呼吸科收治的MP感染患儿35例为观察组,选取同期在儿保科门诊做健康体检的儿童35例为健康对照组。比较两组受试者的T淋巴细胞亚群(CD+3细胞分数、CD+4细胞分数、CD+8细胞分数、CD+4/CD+8细胞比值),肺功能指标[第1秒用力呼气容积(FEV_1)、用力肺活量(FVC)、呼气高峰流量(PEFR)、用力呼气25%流速(MEF25)及用力呼气50%流速(MEF50)],并分析其相关性。结果两组受试者CD+4细胞分数、CD+8细胞分数、CD+4/CD+8细胞比值比较,差异无统计学意义(P>0.05);观察组患儿CD+3细胞分数低于健康对照组(P<0.05)。观察组患儿FEV_1、FVC、PEFR、MEF25、MEF50低于健康对照组(P<0.05)。Pearson相关性分析显示,CD+3细胞分数与FEV_1、PEFR、MEF25、MEF50呈正相关,与FVC无相关性。结论 MP感染患儿可引起细胞免疫紊乱及肺功能下降,且两者之间存在一定的相关性。 Objective To investigate the influence of mycoplasma pneumoniae( MP) infection on cellular immunity and pulmonary function and the correlation analysis. Methods 35 cases of MP infection children were selected from November2013 to January 2014 in Department of Pneumology Children' s Hospital of Suzhou University as observation group,35 cases of healthy children were selected in child care clinic as healthy control group. Two groups of T lymphocyte subsets( CD+3cell fraction、CD+4cell fraction、CD+8cell fraction、CD+4/CD+8cell ratio),pulmonary function index [forced expiratory volume in one second( FEV_1) 、forced vital capcacity( FVC) 、peak expiratory flow rate( PEFR),forced expiratory flow from 25% of FVC( MEF25),forced expiratory flow from 50% of FVC( MEF50) ]were compared,and analyzed its correlation. Results No statistically significant differences of CD+4cell fraction,CD+8cell fraction,CD+4/CD+8cell ratio was found between the two groups( P〈0. 05); observation group of CD+3cell fraction was lower than healthy control group( P〈0. 05). The observation group of FEV_1,FVC,PEFR,MEF25,MEF50 were lower than healthy control group( P〈0. 05). Pearson correlation analysis showed that CD+3cell fraction was positively correlated with FEV_1,PEFR,MEF25,MEF50,but no correlation with FVC. Conclusion MP infected children can cause cell immune disorders and decreased pulmonary function,and there is a certain correlation between the them.
