摘要
目的探讨无创呼吸机治疗新生儿呼吸衰竭的临床疗效。方法选取横县人民医院2013年1月—2016年1月收治的呼吸衰竭患儿76例,随机分为对照组和试验组,各38例。患儿入院后均给予常规治疗,对照组患儿给予机械通气合并气管插管治疗,试验组患儿给予无创呼吸机持续气道正压通气治疗。比较两组患儿的临床疗效,治疗前及治疗后24 h动脉血气分析指标[血氧分压(PaO_2)、血氧饱和度(SaO_2)、血二氧化碳分压(PaCO_2)及pH值]及并发症发生情况。结果试验组患儿临床疗效与对照组比较,差异无统计学意义(P>0.05)。治疗前及治疗后24 h,两组患儿PaO_2、SaO_2、PaCO_2及pH值比较,差异无统计学意义(P>0.05);治疗后,两组患儿PaO_2、SaO_2高于治疗前,PaCO_2低于治疗前(P<0.05),两组患儿治疗前后pH值比较,差异无统计学意义(P>0.05)。试验组患儿治疗期间并发症发生率低于对照组(P<0.05)。结论无创通气治疗新生儿呼吸衰竭与机械通气治疗疗效相当,但其可明显降低感染并发症发生率,安全性好。
Objective To discuss the clinical efficacy of non-invasive ventilation in the treatment of respiratory failure of newborn. Methods A total of 76 cases of respiratory failure of newborn were selected from January 2013 to January 2016 in Hengxian People' s Hospital, which were randomly divided into control group and experimental group,38 cases in each group. All children were given conventional treatment,the control group was treated with mechanical ventilation with tracheal intubation,experimental group was treated with non-invasive ventilation. Clinical efficacy, arterial blood gas analysis index( PaO_2,SaO_2,PaCO_2,pH) before and after treatment of 24 h and the incidence of complications were compared between the two groups. Results The clinical effect of the control group was fit with experimental group( P〈0. 05). Before and after treatment of 24 h,no significant difference of PaO_2,SaO_2,PaCO_2 or pH was found between the two groups( P〈0. 05); after treatment,the two groups of PaO_2,SaO_2 were higher than prior treatment,PaCO_2 were lower than prior treatment( P〈0. 05),no significant difference of pH before and after treatment between the two groups( P〈0. 05). Experimental group of the incidence of complications was lower than control group( P〈0. 05). Conclusion Non-invasive ventilation in the treatment of neonatal respiratory failure has a same significant effect with mechanical ventilation,but it can lower the incidence of complications,and with good safety.
