超声造影联合肿瘤标志物对卵巢良恶性肿瘤的诊断价值

Diagnostic value of contrast-enhanced ultrasound combined tumor marker in predicting benign and malignant ovarian tumors

目的评价超声造影联合肿瘤标志物检测对卵巢良恶性肿瘤的诊断价值。方法选取50例经超声检查有卵巢肿瘤患者作为研究对象,经临床确诊的Ⅰ、Ⅱ、Ⅲ和Ⅳ期卵巢癌患者共15例,经临床病理诊断为卵巢良性肿瘤的患者35例,手术前分别对患者实施超声造影检查、肿瘤标志物检测、联合检查,并对3种临床检查方式的准确性进行评价。结果 1与卵巢良性肿瘤相比,恶性肿瘤表现为不均匀增强,造影强化的始增时间及达峰时间较早,增强速率较高。2卵巢恶性肿瘤组血清糖类抗原(CA)125、CA199、癌胚抗原(CEA)及甲胎蛋白(AFP)结果均显著高于良性肿瘤组,差异有统计学意义(P<0.05)。3肿瘤标志物联合检测的敏感性、特异性、准确性分别为87%(13/15)、89%(31/35)、90%(45/50),都高于单项检测结果。结论超声造影及肿瘤标志物均有助于卵巢恶性肿瘤的检出,采用超声造影及肿瘤标志物联合检测能够显著提高临床检测准确率,为临床诊断及治疗提供有力依据。

Objective To study the diagnostic value of contrast-enhanced ultrasound combined tumor marker in predicting benign and malignant ovarian tumors. Methods Fifty patients with ovarian tumors by ultrasound examination were selected as the research object,15 patients with ovarian cancer were selected as experimental group and 35 cases with benign ovarian tumor as control group. Fifty patients were examined by contrast-enhanced ultrasound,tumor marker and combined detection before surgery,and evaluated the accuracy of the three clinical examinations. Results 1Compared with ovarian benign tumor,malignant tumor was uneven enhancement,the arrival time and the peak time was earlier,the time intensity curve was relatively steep rise. 2 Results of carbohydrate antigen（CA）125,CA199,carcinoembryonic antigen（CEA） and α-fetoprotein（AFP） in ovarian malignant tumor group were significantly higher than the positive control group（P〈0.05）. 3 The sensitivity,specificity and accuracy in combined measur-ment of tumor markers was 87%（13/15）,89%（31/35）,90%（45/50）,obviously higher than that detecting seperately. Conclusion Ultrasonic imaging and tumor markers are contributed to the detection of ovarian malignant tumors. The contrast-enhanced ultrasound and tumor markers combined detection can significantly improve the clinical detection accuracy, which has great significance for clinical diagnosis and treatment.
