血管紧张素Ⅱ对肺性脑病早期诊断价值的探讨

摘要 目的研究慢性阻塞性肺病(COPD)患者血管紧张素Ⅱ(AngⅡ)与肺性脑病(PE)的相关性,探讨AngⅡ对PE早期诊断的意义。方法收集2015年1月-2016年7月COPD并发PE患者75例(PE组)、COPD单纯低氧患者35例(低氧血症组)及同期健康体检者30例(健康对照组),检测入院24h内患者及健康体检者AngⅡ水平、CRP水平、血气分析,并进行肺功能检测。结果与健康对照者比较,PE患者与单纯低氧患者AngⅡ均明显升高,差异有统计学意义(P〈0.05);AngⅡ与二氧化碳分压(PaCO2)呈正相关(r=0.46,P〈0.01),而与血氧分压(PaO2)呈负相关(r=0.57,P〈0.01)。结论AngⅡ水平与COPD缺氧程度呈正相关,可以用于COPD患者PE早期诊断,AngⅡ浓度水平可以有效地评估PE患者病情严重程度及预后。 Objective Through the study of the correlation between Angiotensin Ⅱ and PE, in order to explore the early diagnostic value of AT Ⅱ in pulmonary encephalopathy (PE). Methods Patients from January 2015 to July 2016 were enrolled, 75 cases of patients with pulmonary encephalopathy ( PE group ) , 35 cases with hypoxia alone and 30 cases of healthy check-up, patients and healthy check-up were examined the blood angiotensin Ⅱ ( ang T ) Ⅱ levels, C-reactive protein levels and arterial blood gas analysis, pulmonary function examination within 24 hours after inpatient.All patients were treated according to COPD treatment guidelines for standard treatment. Results The blood levels ofAng Ⅱ rised in both PE and hypoxia patients, and were higher than the health control ( P〈0.05 ) , and the levels ofAng Ⅱ is positively correlated with PaCO2, difference ( r=0.46, P〈0.01 ) , and negatively correlated with PaO2 ( r=0.57, P〈0.01 ) . Conclusions The level of blood AT Ⅱ is positively correlated with the extent of hypoxia in PE, and it could also effectively evaluate the severity of patient's illness and prognosis.
