摘要
目的了解东莞市长安镇2004~2015年〈1岁婴儿麻疹住院病例的住院费用构成情况,探讨住院费用的影响因素,为麻疹防治和卫生决策提供依据。方法采用描述性分析方法分析〈1岁婴儿麻疹住院病例特征和住院费用构成情况,运用多元线性回归分析住院费用的主要影响因素。结果共调查有效病例490例,其中73.06%的病例集中在5~8月。人均住院天数为6.47d,人均住院费用为2 175.63元,占住院费用比例前3位的分别是化验检查费(62.10%)、药费(16.69%)和综合护理费(9.50%)。多元线性回归分析显示住院天数、合并症和月龄的标准偏回归系数分别为0.475(95%CI:0.418,0.532),0.448(95%CI:0.391,0.505)以及-0.052(95%CI:-0.103,-0.001)。结论住院天数、合并症和月龄是影响〈1岁婴儿麻疹住院病例住院费用的主要因素;缩短住院天数、避免合并症和对低月龄患者及早干预,可有效减少〈1岁婴儿麻疹患者的住院费用。
Objective To understand the medical costs and its constitution associated with hospitalization for infants under 1 year old with measles in Chang'an town,2004-2015 ,and explore influencing factors of the medical costs,so as to provide the basis for measles control and health decision. Methods Descriptive analysis method was used to analyze features of hospitalized infants under 1 year old with measles and constitution of hospitalization expenses,and multiple linear regression was used to analyze the influencing factors. Results A total of 490 cases were surveyed,and 73.06% among them occurred during May-August. The average length of hospital stay was 6.47 days. The median of hospitalization cost was 2 175.63 RMB Yuan. The top three items accounting for hospitalization expenses were examination(62. 10 % ) ,medication(16.69%) and nursing care (9.50%) respectively. Multiple linear regression analysis indicated that standard partial regression coefficients of length of hospital stay, complications and age in month of baby were 0. 475(95% CI :0. 418, 0.532),0.448(95% CI :0.391,0.505) and-0.052(95% CI :-0.103,-0.001) respectively. Conclusion Days of hospitalization, complications and age in month of baby, are the main influencing factors on hospitalization costs. Measures on shortening the length of hospital stay, avoiding complications and intervening early on younger infants under 1 year old with measles can effectively reduce hospitalization expenses.
