摘要
目的:观察电针百会穴对内淋巴积水豚鼠耳蜗形态学和血生化的影响,探讨针刺治疗梅尼埃病可能的作用机制。方法:以腹腔注射血管加压素建立内淋巴积水豚鼠模型。40只豚鼠随机分为假手术组、模型组、穴位组(电针百会穴)、非穴位组(电针股部非穴位)、药物组(血管加压素拮抗剂OPC-41061灌胃)及针药联合组。干预28 d后,采用生化、组织形态学观察豚鼠耳蜗内淋巴积水、前庭耳蜗功能和形态的改变。结果:使用血管加压素以及电针或OPC-41061对豚鼠的主要器官和组织未产生明显影响,各组豚鼠血液中生化指标差异均无统计学意义(P>0.05)。HE染色结果显示,穴位组较非穴位组前庭膜膨隆明显减轻,内淋巴积水减少,与OPC-41061的效果相似。顶转、第二转和底转中的前庭阶占前庭阶和中阶的面积之比[S前庭阶/(S前庭阶+S中阶)]模型组明显低于假手术组(P<0.05);穴位组、药物组和针药联合组明显高于模型组(P<0.05);穴位组与非穴位组比较差异有统计学意义(P<0.05)。结论:电针百会穴能明显改善由血管加压素诱导的豚鼠内淋巴积水,提示针刺百会穴可能是通过血管加压素信号途径改善豚鼠内淋巴积水。
Objective: To observe the effects of electro-acupuncture (EA) of Baihui (DU20) acupoint on cochlea morphology and blood biochemical index in guinea pigs with endolymphatic hydrops( EH), and explore the possible mechanism of acupuncture in the treatment of Meniere' s disease. Methods : EH model in guinea pigs was established by intraperitoneal injection of arginine vasopressin (AVP). 40 guinea pigs were randomly divided into the sham-operation group, model group, acupoint group( EA on Baihui acupoint), non-acupoint group( EA on non-acupoint), drug group( AVP antagonist OPC-41061 for intragastric administration) , combination group of EA and drug. After intervention for 28 days, the changes of the function and morphology of EH and vestibular cochlea in guinea pigs were observed and compared by biochemistry and histomorphology. Results: The treatment of AVP,EA and OPC-41061 showed no significant effects on the main organs and tissues of the guinea pig, there were no statistically significant differences on the biochemical indexes in blood of guinea pigs among groups ( P 〉 0.05 ). Compared with the guinea pigs treated with EA on non-acupoints, HE staining showed that the AVP-induced expansion of vestibular membrane was significantly alleviated in the guinea pigs treated with EA on Baihui acupoint, and the EH was reduced,which similar to the effects of OPC-41061. For the area of scala vestibuli (SV)and sum area of SV and scala media (SM) ,the area ratio( Ssv/( Ssv + SSM ))in the model group was obviously lower than that in the sham-operation group (P 〈 0. 05 ), and this area ratio in the EA group, drug group and combination treatment group was obviously higher than that in the model group( P 〈 0.05 ). There was statistically significant difference on the area ratio between the groups treated with EA on Baihui acupoint and non-acupoint ( P 〈 0. 05 ). Conclusion: Electro-acupuncture on Baihui ( DU20 ) acupoint could significantly improve the AVP-indueed endolymphatic hydrops in guinea pigs, which suggested that EA on Baihui acupoint relieved EH via AVP signaling pathway.
