黔南地区新生儿先天性甲状腺功能减退症影响因素分析

Analysis on the influencing factors of neonatal congenital hypothyroidism in Qiannan area

目的探讨黔南地区影响新生儿先天性甲状腺功能减退症(CH)患病的影响因素。方法收集2012年10月-2015年12月黔南地区筛查诊断为CH的新生儿42例作为病例组,按照1∶3比例随机选取同期健康新生儿126例为对照组,采用自行设计《黔南州新生儿先天性甲状腺功能减退症患病影响因素调查表》进行问卷调查,并对影响新生儿CH患病相关因素进行Logistic多元回归分析。结果病例组新生儿父母亲年龄≥35岁及母亲孕期有先兆流产、妊娠期高血压疾病、妊娠期糖尿病、妊娠期合并甲状腺疾病、不良嗜好、焦虑情绪、甲状腺疾病家族史、慢性疾病史、辐射接触史、重金属接触史、用药史与对照组比较,差异有统计学意义(P<0.05);多因素Logistic回归分析结果显示:新生儿父母亲年龄≥35岁及母亲有甲状腺疾病家族史、焦虑情绪、不良嗜好、妊娠期合并甲状腺疾病、用药史和辐射接触史是黔南地区新生儿CH患病的主要危险因素。结论黔南地区新生儿CH患病主要危险因素有父母亲年龄≥35岁和母亲有甲状腺疾病家族史、焦虑、妊娠期合并甲状腺疾病、用药史和辐射接触史等。

Objective To explore the influencing factors of prevalence of neonatal congenital hypothyroidism（ CH） in Qiannan area.Methods Forty-two neonates diagnosed as CH were collected as case group from Qiannan area from October 2012 to December 2015,126 healthy neonates during the same period were randomly selected as control group according to 1 ∶ 3 ratio; a self-designed questionnaire was used to survey the neonates, the influencing factors of CH in neonates were analyzed by multivariate logistic regression analysis.Results There were statistically significant differences in the proportion of neonates with parents＇ age≥35 years old,maternal threatened abortion during pregnancy,hypertensive disorder complicating pregnancy,gestational diabetes,gestational thyroid diseases,bad habits,anxiety,thyroid disease family history,history of chronic diseases,radiation exposure history,heavy metal contact history,drugs use history between the two groups（ P〈0.05）; multivariate logistic regression analysis showed that parents＇ age≥35 years old,family history of maternal thyroid diseases,anxiety,bad habits,maternal gestational thyroid diseases,drugs use history and radiation exposure history were main risk factors of CH in neonates.Conclusion The main risk factors of CH in neonates in Qiannan area included parents＇ age≥35 years old,family history of maternal gestational thyroid diseases,anxiety,bad habits,maternal gestational thyroid diseases,drugs use history and radiation exposure history.
