五加生化胶囊联合卡前列素氨丁三醇注射液治疗产后出血的临床研究

Clinical study on Wujia Shenghua Capsules combined with Carboprost Tromethamine Injection in treatment of postpartum hemorrhage

目的探讨五加生化胶囊联合卡前列素氨丁三醇注射液治疗产后出血的临床疗效,为临床应用提供参考。方法选取2015年9月—2016年9月在三门峡市中心医院接受治疗的产后出血患者84例,按照治疗方法的差别分成对照组(42例)和治疗组(42例)。对照组患者肌肉注射卡前列素氨丁三醇注射液,250μg/次,间隔90 min给药1次,必要时可缩短时间间隔,但不少于15 min,总剂量不超过2 mg。治疗组患者在对照组的基础上口服五加生化胶囊,6粒/次,2次/d。观察两组的临床疗效,比较两组产后出血量、止血时间、一氧化氮(NO)和脑钠肽(BNP)的情况。结果治疗后,对照组和治疗组总有效率分别为80.95%和97.62%,两组总有效率比较具有统计学意义(P<0.05)。治疗2、24 h后,治疗组产后出血量均明显下降,止血时间明显缩短,两组比较差异具有统计学意义(P<0.05)。治疗后,两组血清NO、BNP水平均降低,同组治疗前后差异具有统计学意义(P<0.05);且治疗组上述指标的改善程度更显著,两组比较差异具有统计学意义(P<0.05)。结论五加生化胶囊联合卡前列素氨丁三醇注射液治疗产后出血的临床效果显著,具有很好的止血作用,具有一定的临床推广应用价值。

Objective To explore the clinical effect ofWujia Shenghua Capsules combined with Carboprost Tromethamine Injection in treatment of postpartum hemorrhage, in order to provide reference for clinical use. Methods Patients （84 cases） with postpartum hemorrhage in Sanmenxia Central Hospital from September 2015 to September 2016 were divided into control group （42 cases） and treatment group （42 cases） based on different treatments. Patients in the control group were im administered with Carboprost Tromethamine Injection, 250 μg/time, once every 90 min, if necessary the interval could be shortened, but it could not less than 15 min, and total dose did not more than 2 mg. Patients in the treatment group werepo administered with Wujia Shenghua Capsules on the basis of the control group, 6 grains/time, twice daily. After treatment, the clinical efficacy was evaluated, and the amount of bleeding and haemostatic time, NO, and BNP in two groups before and after treatment were compared. Results After treatment, the clinical efficacies in the control and treatment groups were 80.95% and 97.62%, respectively, and there were differences between two groups （P 〈 0.05）. After treatment for 2 and 24 h, the amount of bleeding in the treatment group was significantly less than that in the control group （P 〈 0.05）. After treatment, NO and BNP in two groups were decreased with significant difference （P 〈 0.05）. And these indicators in the treatment group were improved more significantly than those in the control group （P 〈 0.05）. Conclusion Wujia Shenghua Capsules combined with Carboprost Tromethamine Injection has a significant clinical efficacy in treatment of postpartum hemorrhage with good hemostatic effect, which has a certain clinical application value.
