腔内悬吊联合置管引流治疗瘘管性脓肿

Curative effect observation of suspended cavity and catheter drainage method in treating fistulous abscess

目的观察腔内悬吊联合置管引流治疗瘘管性脓肿的临床疗效。方法将40例瘘管性脓肿患者随机分为治疗组和对照组,各20例。治疗组采用腔内悬吊联合置管引流术,对照组采用低位切开高位挂线术。观察2组患治愈时间、治愈率、术后疼痛程度、评价2组肛门功能及随访3个月后的复发情况。结果 2组治愈率及复发情况比较,差异无统计学意义(P>0.05);治疗组治愈时间较对照组明显缩短(P<0.05);治疗组术后疼痛低于对照组(P<0.05);出院时及随访3个月后,2组肛管最长收缩时间比较,差异无统计学意义(P>0.05);治疗组肛管静息压、直肠静息压及肛管最大收缩压较对照组升高(P<0.05)。结论腔内悬吊联合置管引流术治疗瘘管性脓肿可缩短治愈时间,缓解术后疼痛,较好地保护肛门功能。

Objective To observe the curative effect of fistulous abscesses by hung suspended cavity and catheter drainage method. Method Fistulous abscess patients met the random number table of 40 cases were randomly divided into treatment group and control group with each 20 cases. The treatment grouped was applied to hung suspended cavity and catheter drainage method, the control group was applied with low cut high position hang above method. By observing in dicators including the wound healing time, the cure rate, postoperative pain, anal function and recurrence rate after three months between the two methods. Results The difference was not statistically significant between the two groups about cure rate and recurrence rate（P〉0.05）. The healing time of treatment group was significantly shorter than control group（P〈0.05）; The pain in the treatment group was lower than that in the control group（P〈0.05）; Anal contraction time followed up for three months with discharged patients have no statistical difference（P〉0.05）, but treatment group have statistical difference in anal resting pressure, rectum resting pressure and anal maximum systolic blood pressure compared with control group（P〈0.05）. Conclusion Combination of hung suspended eavity and catheter drainage method have obvious advantages in shortening the healing time, postoperative pain relief and better in protecting anal function.
