绝经后女性高血压患者性激素与胰岛素抵抗的相关性研究 被引量:1

The Relationship Between Sex Hormone and Insulin Resistance Levels in Postmenopausal Women with Hypertension

摘要 目的探讨绝经后女性高血压患者性激素与胰岛素抵抗(IR)的相关性。方法化学发光免疫分析和生化法测定89例绝经后女性高血压患者和64例非高血压患者以及78名对照组血清性激素[催乳激素(PRL)、卵泡生成激素(FSH)、黄体生成激素(LH)、雌二醇(E2)]、空腹血糖和空腹胰岛素水平,计算稳态型胰岛素抵抗指数(HOMA.IR),并进行对比性分析。结果在性激素中,绝经后女性高血压患者血清PRL、FSH和LH水平明显高于非高血压患者(P〈0.05或P〈0.01),血清E2水平明显低于非高血压患者(P〈0.01)。绝经后女性高血压患者HOMA-IR水平显著高于非高血压患者(P〈0.05)。结论绝经后女性高血压和性激素代谢紊乱引起高胰岛素血症是产生IR的原因,早期营养干预和绝经后激素替代治疗可以改善IR。 Objective To explore the relationship between sex hormone and insulin resistance (IR) levels in postrnenopausal women with hypertension. Methods The sex hormone (PRL, FSH, LH, E2), fasting blood glucose and fasting insulin levels were determined in 89 postmenopausal women with hypertension, 64 postmenopausal women without hypertension and 78 normal controls by chemiluminescence immunoassay and biochemistry, then HOMA-IR level was calculated, and compared analysis was done between 3 groups. Results In the sex hormone, the serum PRL level was significantly higher in postmenopausal women with hypertension than that of postmenopausal women without hypertension (P 〈 0.05), the serum FSH and LH levels were obviously increased (P 〈 0.01), but the serum E2 level was significantly decreased (P 〈 0.01). The HOMA-IR level was significantly higher in postmenopausal women with hypertension than that in postmenopausal women without hypertension (P 〈 0.05). Conclusion Hypertension in postmenopausal women and sex hormone metabolism disorder was a cause of IR by resulting in hyperinsulinemia, and IR may be improved through early complement nurture and postmenopausal hormone replacement therapy.
