针刺风池穴对视神经萎缩患者图形视觉诱发电位的即刻效应

Immediate effect of acupuncture at Fengchi(GB20) on pattern visual evoked potential in patients with optic atrophy

目的探讨针刺风池穴对视神经萎缩患者图形视觉诱发电位(P-VEP)的即刻影响。方法视神经萎缩患者30例(51只眼),由同一名医师针刺双侧风池穴,进针方向朝向对侧眼球,用热补手法,针刺得气后留针15 min。针刺前及出针即刻采用两种空间频率(1°方格及15′方格)对患眼进行P-VEP检查,比较针刺前后P_(100)波的潜伏期及振幅变化。结果 1°方格:针刺前P_(100)波潜伏期(121.17±14.28)ms,振幅(5.21±4.12)μV,针刺后潜伏期(129.39±19.41)ms,振幅(6.04±4.76)μV。15′方格:针刺前P_(100)波潜伏期(122.29±16.23)ms,振幅(4.32±3.98)μV,针刺后潜伏期(127.90±19.12)ms,振幅(5.30±3.89)μV。针刺后两种空间频率下的P_(100)波潜伏期均较针刺前延长,振幅较针刺前增加,前后差异均有统计学意义(配对t检验,P<0.05)。结论针刺风池穴对视神经萎缩患眼P-VEP存在即刻影响,包括P_(100)波潜伏期延迟及振幅增加,前者体现了针刺风池穴对视皮层的抑制作用,后者可能与针刺风池穴调整了眼部供血有关。

OBJECTIVE To explore the immediate effect of acupuncture at Fengchi （GB20） on pattern visual evoked potential （P-VEP） in optic atrophy （OA）. METHODS Examine 30 cases （51 eyes） of OA patients with P-VEP. One fixed doctor acupunctures on both Fengchi （GB20）, the needles are heading to the opposite side eye and then apply the needling technique of heat and reinforce for 15 rain. Two kinds of space frequency （1°Grid and 15＇ Grid） were carried out before and immediately after the treatment.Calculate the difference of latent time and swings of the P100 wave before and after treatment. RESULTS Before acupuncture, the latent time of P100wave of 1° Grid was 121.17±14.28ms, swings was 5.21±4.12μV, while after acupuncture the results were 129.39±19.41ms and 6.04±4.761μV. In terms of 15＇ grid, latent time of Pl00 wave was 122.29±16.23ms, swings was 4.32±3.98μV, after acupuncture the outcomes were 127.90±19.12ms and 5.30±3.89μV. For both kinds of space frequency, P100 wave latent times were delayed while amplitudes were increased after the therapy. The difference was of statistical signifi- cance （paired t test, P〈0.05）. CONCLUSIONS Acupuncture at Fengchi （GB20） had immediate influence on P-VEP in OA patients, including delayed latent time of P100 wave and increased amplitude. The former reflected the visual cortex inhibition, the latter may be associated with redistribution of blood supply around eye arisen by stimulation of Fengchi （GB20）.
