视网膜神经纤维层厚度与糖尿病周围神经病变的相关性研究 被引量:6

摘要 目的探讨不同程度糖尿病周围神经病变(diabetic peripheral neuropathy,DPN)患者视网膜神经纤维层(retinal nerve fiberlayer,RNFL)厚度的变化。方法选择2014年1月—2016年2月在保定市第二中心医院内分泌科住院治疗的DPN 120例作为观察组,另选择同期在该院体检的健康体检者50例作为对照组。比较两组RNFL厚度以及观察组不同程度患者RNFL厚度。结果观察组视盘上方、下方RNFL厚度小于对照组,平均RNFL厚度大于对照组,两组比较差异有统计学意义(P<0.05)。不同程度DPN患者视盘上方、下方及平均RNFL厚度总体比较差异有统计学意义(P<0.05);不同程度DPN患者随病情加重,视盘上方、下方及平均RNFL厚度渐减小,差异有统计学意义(P<0.05)。结论 DPN程度与RNFL厚度有一定相关性,随着DPN程度加重,RNFL厚度有逐渐变薄趋势。临床可根据RNFL厚度结合实际评估DPN患者发生及病情进展情况。 Objective To investigate changes of retinal nerve fiber layer (RNFL) thickness of diabetic peripheral neuropathy (DPN) patients with different degrees. Methods A total of 120 DPN patients admitted during January 2014 and February 2016 were selected as observation group, and another 50 healthy person taking physical examination at the same peri- od were chosen as control group. Values of RNFL thickness in two groups were compared, and values of RNFL thickness of different degree patients in observation group were also observed. Results In observation group, values of RNFL thickness of upper and lower optic disc were less than those in control group ( P 〈 O. 05 ). There were significant differences in values of RNFL thickness of upper, lower and bitamporal optic disc, and average RNFL thickness in DPN patients with different degrees (P 〈 0. 05 ). Values of RNFL thickness of upper and lower optic disc and the average RNFL thickness were gradually decreased with the disease aggravation in DPN patients with different degrees (P 〈 0.05). Conclusion There is a certain correlation between DPN degree and RNFL thickness. Thickness of RNFL is gradually thinning with DPN aggravation. Pathogen- esy and development of DPN patients can be evaluated according to RNFL thickness combined with clinical conditions.
