基于DEA模型的广东省58家县人民医院效率研究

Efficiency of Hospitals Based on DEA Model: A Case of 58 County People's Hospitals

目的探究广东省58家县人民医院的运行效率,为政府有效投入与医院优化管理提供依据和建议。方法以卫生技术人员数、床位数、总诊疗人次、病床使用率等作为评价医院运行效率的投入产出指标,运用DEAP2.1软件,建立以产出为导向的DEA-VRS模型进行分析评价。结果平均综合效率为0.841,平均技术效率为0.917,平均规模效率为0.914。31家医院的效率为DEA无效,若加强管理,平均可增加总诊疗人次43859.0人次,出院人次387.0人次,平均病床使用率可由83.7%提高至86.8%。结论造成DEA无效的主要原因是万元以上设备冗余较为严重,总诊疗人次数偏低。所以,广东省的县人民医院主要应从提升服务能力,使总诊疗人次增长来提高医院运行的效率。

Objective To provide models and suggestions for the government on effective investment and hospital op-timal management by exploring the operation efficiency of 58 peopled hospitals in the counties of Guangdong Province. Methods The output - oriented DEA - VRS model was created to evaluate the efficiency of hospital operation in DEAP2. 1 with the number of health technical personnel, the number of hospital beds as input indexes and the total number of outpatient visits, bed utilization ratio as output indexes. Results The average overall efficiency, average technical efficiency and average scale efficiency was 0. 841,0. 917 and 0. 914 respectively. The efficiency of 31 hospitals was DEA invalid but if the manage-ment was improved, the total number of outpatient visits and the number of discharged patients could reach an average in-crease of 43,859. 0 and 387. 0, respectively and the average bed utilization ratio could be increased from 83. 7% to 86. 8% . Conclusion The main reasons of DEA invalid results lie in the redundancy of equipment which costs more than 10,000 yuan and the low total number of outpatient visits. Therefore, the peopled hospital administrators in counties of Guangdong Prov-ince should pay main attention to service improvement to enhance the operation efficiency of the hospital by increasing the total number of outpatient visits.
