CerbB-2、Ki-67基因蛋白在胃腺癌中的相关性及微转移中的意义

The Correlation and the Significance of Micro Metastasis between CerbB-2 and Ki-67 Gene Protein Expression in the Gastric Adenocarcinoma

目的研究胃腺癌基因蛋白CerbB-2、Ki-67的临床意义、相关性及与微转移关系。方法应用免疫组织化学法(IHC)对458例胃腺癌组、95例对照组,82例早期黏膜下胃腺癌淋巴标本中CerbB-2、Ki-67基因蛋白进行检测。结果 CerbB-2基因蛋白在胃腺癌组及对照组中的高表达率分别为32.3%、9.47%;Ki-67基因蛋白高增殖活性率则分别为48.7%、17.9%,存在显著性差异;CerbB-2表达与患者年龄、性别、肿瘤大小、分化程度无相关性,而与TNM分期、浸润深度、转移有关;Ki-67的高增殖活性与患者年龄、性别、肿瘤大小无关,而与肿瘤分化程度、TNM分期、浸润深度、转移有关。CerbB-2、Ki-67检测结果具有相关性。82例早期黏膜下胃腺癌患者淋巴结基因蛋白Ki-67、CPK、CerbB-2高表达率分别为42.5%、42.5%、0%,82例中11例转移,27例淋巴结微转移,相比HE染色检出率明显增高。结论 CerbB-2、Ki-67基因蛋白与胃腺癌的发生、发展有明显的关联。Ki-67基因蛋白检测对微转移有意义,对早期发现胃腺癌有着极大意义;而Cerb B-2不明显。

Objective To investigate the correlation and clinical significance of CerbB-2,Ki-67 gene protein in the gastric adenocarcinoma.Methods The CerbB-2 and Ki-67 gene protein were detected in 458 cases of gastric adenocarcinoma tissue,95 cases of the control tissue,82 cases of submucosal gastric adenocarcinoma lymph nodes by using immunohistochemistry.Results The positive rate of CerbB-2 expression in gastric adenocarcinoma tissues group and the control group were 32.3% and 1.05% respectively.The high proliferative activity of Ki-67 in tow groups were 48.7% and 17.9% respectively.There were significantly differences between two groups in the expressions of CerbB-2 and Ki-67 gene protein.The expression of CerbB-2 was not correlated with age,gender,tumor size or tumor differentiation,but was related to TNM stage,depth of invasion and metastasis The high proliferative activity of Ki-67 was not related to age,sex and tumor size,but was related to the degree of tumor differentiation,TNM stage,depth of invasion and metastasis.The results of CerbB-2 and Ki-67 were correlated The gene protein high expression rates of Ki-67,CPK,CerbB-2 in early gastric cancer lymph node in 82 cases with submucosal were 42.5%,42.5% and 0%.There were 11 cases of metastasis and 27 cases of lymph node micrometastasis in 82 cases.The detection rate increased significantly than HE staining.Conclusion CerbB-2 and Ki-67 gene protein can be considered as important index.The detection of Ki-67 gene protein is very significant for micrometastasis,which has great significance for early detection of gastric cancer,but CerbB-2 is not obvious.
