动态监测血乳酸对脓毒症患儿预后状况的预测价值被引量：4

摘要目的探讨动态监测血乳酸对脓毒症患儿预后状况的预测价值。方法选取我院儿科重症监护室收治的56例脓毒症患儿为观察组,其中发生多器官功能障碍综合征(MODS)患儿27例,无MODS患儿29例,存活40例,死亡16例。选取同时期在我院行健康体检的儿童50例设为对照组。对照组儿童于体检时采集0.5mL静脉血,观察组患儿则需于入住儿科重症监护室后1h内(0h),入住后12h、24h、48h采集0.5mL静脉血,并测定其血乳酸水平。结果观察组血乳酸水平明显高于对照组(P<0.05),MODS患儿血乳酸水平明显高于无MODS患儿(P<0.05)。死亡患儿0h、12h、24h、48h时血乳酸水平均明显高于存活患儿(P<0.05),存活患儿血乳酸水平随着治疗时间逐渐下降,而死亡患儿血乳酸水平随着治疗时间逐渐升高。0h时血乳酸水平<2.0 mmol/L、2.0~4.0mmol/L、>4.0mmol/L患儿病死率依次为7.69%、22.22%、44.00%,三组间病死率差异具有统计学意义(P<0.05)。结论动态监测血乳酸有助于准确评估脓毒症患儿病情发展及预后状况。
