经腹部超声联合经阴道超声在妇科急腹症诊断中的价值

The Value of Abdominal Ultrasound and Transvaginal Ultrasound in the Diagnosis of Gynecological Acute Abdomen

目的:研究经腹部超声(TAS)联合经阴道超声(TVS)检查对妇科急腹症的诊断价值。方法:选取滦南县妇幼保健院妇产科收治确诊的150例急腹症患者,均保留完整的病历及影像资料,以手术病理结果为金标准,回顾性分析观察患者行TAS联合TVS检查的诊断结果。结果:单独TAS检查正确诊断率81.33%(122/150),误诊16例,漏诊12例;单独TVS检查正确诊断率84.67%(127/150),误诊13例,漏诊10例;TAS联合TVS检查正确诊断率94.00%(141/150),误诊6例,漏诊3例;单独TAS、TVS检查诊断正确率相较无显著差异(P>0.05),TAS、TVS联合检查诊断正确率明显高于单一TAS或TVS检查,差异均具有统计学意义(P<0.05)。结论:TAS、TVS作为妇科急腹症的常见检查手段,二者联合应用可降低漏诊、误诊风险,提高正确诊断率,为临床治疗及制定处理方案提供可靠依据。

Objective： To Study on abdominal ultrasound （TAS） combined with transvaginal ultrasound （TVS） in diagnosing gynecological acute abdomen. Methods： 150 cases of patients with acute abdomen in our hospital obstetrics and Gynecology Hospital, medical records and imaging data were kept intact, with surgical and pathological results as the gold standard,retrospective analysis of the diagnosis results were observed for TAS combined with TVS examination. Results： The correct diagnostic rate of TAS was 81.33% （122/150）, 16 cases of misdiagnosis, missed diagnosis in 12 cases; the TVS test alone, the accuracy rate was 84.67% （127/150）, 13 cases of misdiagnosis, missed diagnosis in 10 cases; TAS combined with TVS to check the correct diagnosis rate was 94% （141/150）, 6 cases of misdiagnosis, missed diagnosis in 3 cases; Single TAS or TVS, check the rate of correct diagnosis compared with no significant difference （P〉0.05）, TAS, TVS joint inspection the correct diagnosis rate was significantly higher than that of single TAS or TVS examination, the difference was statistically significant （P〈0.05）. Conclusion： TAS and TVS as the common examination of gynecological acute abdomen, the combination of the two can reduce the misdiagnosis risk, improve the correct diagnosis rate for clinical treatment and provide a reliable basis for formulating treatment plan.
