钽棒联合自体骨移植在早期股骨头坏死患者中的应用价值分析

The Application Value of Tantalum Rods Combined with Autogenous Bone Graft in Patients with Early Femoral Head Necrosis

目的探究钽棒联合自体骨移植在早期股骨头坏死患者中的应用价值。方法选取于2010年6月至2012年6月期间我院收治的39例早期股骨头坏死患者,行回顾性病例对照研究,其中观察组20例和对照组19例,对照组行髓芯减压和钽棒植入术,观察组在此基础上联合自体骨移植进行治疗,记录两组患者术中情况、住院时间、VAS评分均降低、Harris评分以及并发症发生情况,并对两组的疗效进行分析比较。结果两组患者术前VAS评分和Harris评分均无显著差异(P>0.05);术后两组患者VAS评分均降低、Harris评分均增高,且组间比较有统计学意义(P<0.05)。观察组并发症发生率为10.71%,对照组并发症发生率为48%,两组比较有统计学意义(P<0.05)。结论钽棒联合自体骨移植治疗早期股骨头坏死疗效较为理想,髋关节功能恢复好,疼痛减轻且不良事件发生概率低。

Objective To explore the application value of tantalum rod combined with autogenous bone graft in the early stage of femoral head necrosis. Methods 39 cases of early femoral head necrosis were selected from June 2010 to June 2012 in our hospital,and the data was retrospectively analyzed. There were 20 cases in the observation group and 19 cases in the control group. Core decompression and tantalum rod implantation was used in the control group,and autogenous bone graft was used in addition in the observation group. It was compared of the clinical outcomes between the two groups including hospitalization time,visual analogue score（ VAS）,Harris score and the incidence of complications. Results There was no significant difference in the preoperative VAS and Harris score between the two groups（ P〉0. 05）. The postoperative VAS and Harris score of the two groups were significantly different（ P〈0. 05）,and the observation group had superiority.The complication rate of the observation group was 10. 71%,which was much lower than 48% of the control group. There was significant difference between the two groups（ P〈0. 05）. Conclusion Tantalum rods combined with autogenous bone graft in the treatment of early avascular necrosis of the femoral head is ideal,with a satisfactory hip function recovery and pain relief. The incidence of adverse events is lower than the treatment of core decompression and tantalum rod implantation group.
