老年急性胆囊炎腹腔镜胆囊切除的临床分析

Clinical analysis of senile acute cholecystitis resection by laparoscope

目的分析将腹腔镜胆囊切除手术应用在急性胆囊炎老年患者治疗中的效果。方法 68例急性胆囊炎老年患者,依照治疗的方式不同分成开腹组与腹腔镜组,各34例。开腹组患者使用开腹手术治疗,腹腔镜组采用腹腔镜胆囊切除手术治疗,对比两组治疗的效果。结果腹腔镜组患者术中出血量(44.36±5.31)ml、手术时间(39.64±4.53)min、下床活动时间(24.38±2.61)h、肠道功能恢复时间(15.26±6.42)h、住院时间(4.32±0.31)d均优于开腹组的(83.42±8.37)ml、(68.54±9.35)min、(36.25±3.52)h、(25.19±6.37)h、(8.76±1.46)d,差异有统计学意义(P<0.05)。腹腔镜组患者并发症发生率为8.82%,低于开腹组的35.29%,差异有统计学意义(P<0.05)。结论将腹腔镜胆囊切除手术应用在急性胆囊炎老年患者治疗中,可以减少患者术中的出血量,缩短手术时间与住院时间,降低并发症的发生率,值得应用。

Objective To analyze effect of laparoscopic cholecystectomy in senile acute cholecystitis patients. Methods A total of 68 senile acute eholecystitis patients were divided by different treatment methods into open surgery group and laparoscopic group, with 34 cases in each group. The open surgery group received open surgery for treatment, and the laparoscopic group received laparoscopic eholeeystectomy for treatment. Therapeutic effect was compared in two groups. Results The laparoseopic group had intraoperative bleeding volume as （44.36 ± 5.31） ml, operation time as （39.64 ± 4.53） rain, time of leaving bed as （24.38 ± 2.61） h, intestinal function recovery time as （15.26 ± 6.42） h, and hospital stays as（4.32 ± 0.31） d, which were all better than （83.42 ± 8.37） ml, （68.54 ± 9.35）min, （36.25 ± 3.52） h, （25.19 ± 6.37） h and （8.76 ± 1.46） d in the open surgery group, and their difference had statistical significance （P〈0.05）. The laparoscopic group had lower incidence of complications as 8.82% than 35.29% in the open surgery group, and the difference had statistical significance （P〈0.05）. Conclusion Implement of laparoscopic cholecystectomy for the treatment of senile acute cholecystitis, can reduce the intraoperative bleeding volume, shorten operation time and hospital stays, decrease incidence of complications. So this method is wroth application.
