生长抑素对ERCP术后大鼠胰腺组织的影响

摘要 目的通过各组大鼠ERCP术后模型的组间比较,研究不同因素对胰腺组织的影响,从而为预防临床ERCP术后胰腺炎的发生提供实验和理论依据。方法 80只Wistar大鼠随机分成4组,每组20只。制造大鼠ERCP术后模型。胰管内注射生理盐水组胰管内注入生理盐水(0.1ml/100g)。胰管内注射30%泛影葡胺组胰管内注入30%泛影葡胺(0.1ml/100g)。胰管内注射50%泛影葡胺组胰管内注入50%泛影葡胺(0.1ml/100g)。思他宁组在模型建立后随即自股静脉脉注入思他宁10μg/100g,之后胰管内注入30%泛影葡胺(0.1ml/100g)。检测术后第4、8、12、24h血清淀粉酶。结果术后第4、8h各组比较:胰管内注射50%泛影葡胺组较胰管内注射30%泛影葡胺组有显著性差异(P<0.05)。胰管内注射30%泛影葡胺组较胰管内注射生理盐水组、思他宁+胰管内注射30%泛影葡胺组之间各时段淀粉酶值都有统计学差异,而胰管内注射生理盐水组、思他宁+胰管内注射30%泛影葡胺组各时段淀粉酶值之间没有统计学差异(P>0.05)。术后第12、24h各组比较:胰管内注射30%泛影葡胺组,胰管内注射50%泛影葡胺组淀粉酶值进行性升高,均较各自前一时段淀粉酶值有显著性差异(P<0.05),此两组之间也有统计学差异(P<0.05)。而胰管内注射生理盐水组、思他宁+胰管内注射30%泛影葡胺组淀粉酶值呈缓坡样增长,均较各自前一时段淀粉酶值无统计学差异(P>0.05),此两组之间各时段比较均无统计学差异。结论 (1)泛影葡胺对胰腺组织有损伤作用,并且随着浓度升高损伤作用加强;(2)小剂量思他宁对ERCP术后胰腺炎有预防作用。为研究预防ERCP术后胰腺炎的发生打下了基础。 Objective To establish a post - ERCP model in rats and compared the model with group com-parison ,to investigate the influence that different factors affect pancreatic tissue ,in attempt to provide experi-mential and theoretical references for preventing clinical post - ERCP pancreatitis .Method 8 0 SD rsts were randomly allocated into 4 groups ,2 0 per group .establishing a post - ERCP model in rats .the only differerce from each group was medicine .the group of using Stilamin were infuse with Stilamin from femoral vein in a dose of 1 0 ug/1 0 0 g ofter the model was established ,than infuse with5 0 % Acetrizoic Acid(0 .1 ml/1 0 0 g) .serum amylase were detected at 4 、8 、1 2 、2 4 h after post - ERCP .Result There were significant differences in the group of 5 0 % Acetrizoic Acid compared with 3 0 % ,there were significant differences in the group of 3 0 % Acetri-zoic Acid compared with the rest two groups ,there were no statistics diversity in the last two groups .Compared with other groups in 1 2 、2 4 h post - ERCP :serum amylase were advanced gradually in consecutive sequence of the group of 3 0 % Acetrizoic Acid and the group of 5 0 % Acetrizoic Acid ,the target was higher than the former (P〈 0 .0 5 )and there were significant difference in the two groups( P 〈 0 .0 5 ) .there were no statistics differ-ences in the last two groups compared with the former serum amylase although the target increased slowly(P 〉0 .0 5 ) ,there were also no statistics differences compared with the last two groups .The tendency of patho - clas-sify score approach to the diversify of serum amylase .Conclusions (1 )Acetrizoic Acid could do harm to pancre-atic tissue ,and the damage were strengthened followed by higher density ;(2 ) minidose of Stilamin in applied be-fore ERCP can effectively prevent the occurrence of postoperative pancreatitis .
