胰岛素泵在非小细胞肺癌合并糖尿病患者围手术期中的应用研究 被引量:1

摘要 目的探讨胰岛素泵在非小细胞肺癌合并糖尿病患者围手术期中的应用研究。方法回顾我院2015年1月至12月收治的非小细胞肺癌合并糖尿病的手术患者127例,根据胰岛素应用方式的不同分为观察组和对照组。对照组采用多次皮下注射胰岛素控制血糖,观察组采用胰岛素泵皮下持续泵入胰岛素控制血糖,比较两组血糖控制水平、胰岛素用量、术后康复情况及住院费用等。结果观察组血糖达标时间短于对照组,治疗后日平均血糖低于对照组,胰岛素日用量少于对照组,血糖异常率低于对照组,差异有统计学意义(P<0.05)。观察组的手术等待时间、抗生素使用时间、住院时间均短于对照组,术后并发症、住院费用少于对照组,差异有统计学意义(P<0.05)。结论胰岛素泵能够提高非小细胞肺癌合并糖尿病患者围手术期的血糖控制水平,减少术后并发症,促进患者康复,且不会增加住院费用,值得推广。 Objective To investigate the application of insulin pump in the perioperative of non-small cell lung cancer with diabetes mellitus.Methods 127 cases of non-small cell lung cancer with diabetes mellitus in our hospital from January to Decem- ber 2015 were divided into the observation group and the control group according to the different insulin application.The control group was subcutaneous injected insulin to control blood glucose, and the observation group was injected insulin with insulin pump. The two groups were compared with the blood glucose control level,insulin dosage, hospitalization expenses and postoperative re- covery.Results The time of reaching satisfied glucose level in the observation group was shorter than that in the control group.After insulin pump treatment average blood glucose, insulin dosage and blood glucose abnormality rate were lower than that in the control group.Similarly the preoperative waiting time, duration of antibiotics use and hospital stay were also shorter than that in the control group.Meanwhile postoperative complications,hospitalization costs were less than those in the control group.Conclusion Insulin pump can improve the perioperative blood glucose control level in non-small cell lung cancer with diabetes mellitus,reduce the postoperative complications, promote the rehabilitation, and not increase the cost of hospitalization.
