摘要
目的观察胆管内放置曲安奈德可降解胆道支架后胆管ɑ平滑肌肌动蛋白(ɑ-SMA)及转化因子β_1(TGF-β_1)在兔胆管损伤动物模型修复期间的表达情况,研究应用曲安奈德药物覆膜的可降解胆道支架在兔胆管损伤动物模型中抑制胆管炎性狭窄的机制。方法通过兔胆管损伤动物模型将应用曲安奈德药物涂层的可降解胆道支架和无曲安奈德涂层可降解胆道支架分别放置兔肝外胆管60只,平均随机分成6组,选取放置胆道支架后第30天、60天、90天分别处死1组兔动物模型,取材兔损伤修复模型的胆管并且应用免疫组织化学的方法分别检测ɑ-SMA和TGF-β_1蛋白的表达水平。结果曲安奈德可降解胆道支架组兔胆管SMA-ɑ和TGF-β_1与无曲安奈德涂层可降解胆道支架组相比表达减弱,曲安奈德可降解胆道支架组兔胆管狭窄,发生比无曲安奈德涂层可降解胆道支架组低。曲安奈德可降解胆道支架组第30天与第90天二组有差异性统计学意义(P<0.05),无曲安奈德涂层可降解胆道支架组第60天与第90天组二组有差异性统计学意义(P<0.05)。结论曲安奈德药物覆膜可降解支架在兔胆管损伤动物模型修复期间胆管组织中ɑ-SMA和TGF-β_1两种蛋白表达量降低,提示曲安奈德药物覆膜可降解支架在抑制兔胆管损伤动物模型的炎性狭窄起到一定作用。
Objective To investigate the effect of a biodegradable triamcinolone-eluting biliary polylactide stent on the expression-smooth muscle actin(ɑ-SMA)and transforming growth factor-bata1(TGF-β1)in leporine bile duct after injury.Methods The triamcinolone-eluting biliary polylactide stents were prepared by immersion method.A leporine bile duct injury model was established.Triamcinolone-eluting biliary polylactide stents were surgically inserted in the bile duce of 30 leporines in experimental group,while biodegradable without triamcinolone stent were placed in 30 leporines of control group.Animals were sacrificed 30,60,90 days after operation.The wound healing of common bile duct was observed.The expression ofɑ-SMA and TGF-β1 was detected with immunohistochemical methon.Results Theɑ-SMA and TGF-β1 were expressed lower than those in control group(P〈0.05).In experimental group,theɑ-SMA and TGF-β1 were expressed lower than those in control group on 30 days and 90days(P〈0.05).In control group,theɑ-SMA and TGF-β1 were expressed higher than those in experimental group on 60 days and 90days(P〈0.05).Conclusion Local delivery of triamcinolone via a biodegradable stent can decrease the expression ofɑ-SMA and TGF-β1in healing process of injuried leporine bile duct,which may be potentially relieve the biliary stricture.
