幽门螺杆菌感染与消化性溃疡患者血清中IL-6、IL-8、IL-2相关性临床观察

Peptic ulcer with Hp infection and its relationship with serum IL-6,IL-8 and IL-2

目的探讨消化性溃疡幽门螺杆菌(Hp)感染与血清中IL-6、IL-8、IL-2的关系,了解血清细胞因子在Hp相关消化性溃疡发病过程中免疫反应的表达,以及抗Hp治疗后上述指标的变化。方法回顾性分析自2014年7月至2015年1月解放军180医院收治的88例消化性溃疡患者的临床资料。其中,55例患者为Hp阳性(Hp阳性组),33例患者为Hp阴性(Hp阴性组)。所有患者在治疗前后均进行血清IL-6、IL-8及IL-2检测。结果 Hp阳性组治疗前血清IL-6、IL-8水平明显高于Hp阴性组,差异有统计学意义(P<0.05);Hp阳性组治疗前血清IL-2水平明显低于Hp阴性组,差异有统计学意义(P<0.05);治疗后,Hp阳性组血清IL-2、IL-6、IL-8水平均接近于Hp阴性组,差异无统计学意义(P>0.05);治疗后,Hp阳性组血清IL-2、IL-6、IL-8水平与治疗前比较,差异有统计学意义(P<0.05)。结论血清IL-2、IL-6、IL-8水平的改变可以抑制Hp感染机体的细胞免疫,激活体液免疫,其水平的高低可作为判定机体免疫情况的一个指标。

Objective To investigate the relationship between the Hp infection in patients with peptic ulcer and serum IL-6,IL-8 and IL-2,to understand the immunoreaction expression of serum cytokine during the pathogenic process of Hp-related peptic ulcer as well as the changes in above indexes after the anti-Hp therapy. Methods A retrospective study was performed on 88 patients with peptic ulcer admitted in No. 180 Hospital of PLA from July 2014 to January 2015. Among the patients,55 cases were Hp positive（ Hp positive group）,33 cases were Hp negative（ Hp negative group）. All the patients were detected for serum IL-6,IL-8 and IL-2 before and after receiving the standard treatment and Hp radical treatment. Results The levels of serum IL-6 and IL-8 of Hp positive group before treatment were significantly higher than those of Hp negative group（ P〈0. 05）; the level of IL-2 in the Hp positive group before treatment was significantly lower than that in the Hp negative group（ P〈0. 05）; after treatment,the contents of serum IL-2,IL-6 and IL-8of Hp positive group were close to those of Hp negative group（ P〉0. 05）; after the treatment,the contents of serum IL-2,IL-6 and IL-8 of Hp positive group were significantly different from those before treatment（ P〈0. 05）. Conclusion Hp infection could inhibit the cellular immunity of organism and activate the humoral immunity,which level can be regarded as an index to determine the condition of organism immunity.
