枸橼酸咖啡因注射液治疗早产儿呼吸暂停的疗效观察 被引量:11

摘要 目的:探讨枸橼酸咖啡因注射液治疗早产儿呼吸暂停的临床效果。方法:选择56例呼吸暂停早产儿,按照随机数字表法分为观察组和对照组,每组各28例。观察组患儿给予枸橼酸咖啡因注射液,对照组患儿给予氨茶碱注射液。比较2组患儿治疗后的呼吸暂停发作次数、呼吸暂停消失时间及氧疗时间,治疗前后采用中国儿童发展中心婴幼儿智力量表评估智力发育指数(mental development index,MDI)与运动发育指数(psychomotive development index,PDI)。结果:治疗后,观察组患儿治疗48 h内呼吸暂停发作次数明显减少,呼吸暂停消失时间、氧疗时间明显缩短,与对照组比较,差异有统计学意义(P<0.05)。治疗前,2组患儿MDI、PDI评分的差异无统计学意义(P>0.05);治疗后,观察组患儿MDI、PDI评分均明显高于对照组,差异均有统计学意义(P<0.05)。观察组患儿的总有效率为89.3%(25/28),明显高于对照组的64.3%(18/28),差异有统计学意义(P<0.05)。结论:采用枸橼酸咖啡因注射液治疗早产儿呼吸暂停,可明显缩短氧疗时间,促进患儿脑神经发育,疗效较好。 OBJECTIVE: To probe into the clinical effects of caffeine citrate injections in treatment of apnea of prematurity. METHODS: 56 cases of apnea of prematurity were selected to be divided into observation group and control group via the random number table,with 28 cases in each. The observation group were treated with caffeine citrate injections,while the control group were given aminophylline injections. The number of apnea episodes,the duration of apnea and oxygen therapy time of two groups were compared,and the intelligence development index( MDI) and motor development index( PDI) of two groups were evaluated before and after treatment. RESULTS: After treatment,the number of apnea episodes within treatment 48 h in observation group decreased significantly,and the duration of apnea and oxygen therapy time were shortened; compared with the control group,the difference was statistically significant( P〈0. 05). Before treatment,there was no significant difference in MDI and PDI scores between the 2 groups( P〈0. 05); after treatment,the MDI and PDI scores of the observation group were significantly higher than those of the control group,with statistically significant difference( P〈0. 05). The total effective rate of observation group was 89. 3%( 25 /28),significantly higher than that of control group[64. 3%( 18 /28) ],with statistically significant difference( P〈0. 05). CONCLUSIONS: Caffeine citrate injections in treatment of apnea of prematurity can significantly shorten oxygen therapy time,promote children's brain development with remarkable efficacy.
