干预前后焦作市第二人民医院住院患者糖皮质激素应用分析

Analysis on Application of Glucocorticoids in Inpatients from Jiaozuo Municipal the Second People's Hospital Before and After Intervention

目的:探讨实施糖皮质激素合理应用综合评价和干预措施对焦作市第二人民医院(以下简称"我院")糖皮质激素合理应用的影响。方法:调取我院2014年1—6月576例使用糖皮质激素住院患者(干预前)的归档病历,针对糖皮质激素不合理应用的问题进行干预;调取2014年7—12月645例使用糖皮质激素住院患者(干预后)的归档病历,对比分析干预前后糖皮质激素的使用情况及不合理用药情况。结果:干预前糖皮质激素不合理使用率为13.37%(77/576),干预后降至4.50%(29/645),降低了8.87%,且干预后用药选择不适宜率、剂量不适宜率、联合用药不适宜率及其他不合理用药率均明显低于干预前,差异均有统计学意义(P<0.05)。结论:我院对糖皮质激素不合理应用问题实施的干预措施,可进一步规范糖皮质激素的临床使用。

OBJECTIVE： To probe into the effects of implementation of glucocorticoids comprehensive assessment and intervention on rational application of glucocoticoids in Jiaozuo Municipal the Second People＇s Hospital（ hereinafter referred to as ＂our hospital ＂）. METHODS： Archival records of 576 inpatients with glucocoticoids（ before intervention） were extracted from Jan. to Jun. 2014,the irrational application of glucocoticoids were intervened. And645 inpatients with glucocoticoids（ after intervention） were extracted from Jul. to Dec. 2014 were extracted,the application status and irrational application of glucocoticoids before and after intervention were analyzed. RESULTS：The irrational application rate of glucocoticoids was 13. 37%（ 77 /576） before intervention,which decreased to 4. 50%（ 29 /645） after intervention,decreased by 8. 87%; and the rate of improper drug selection,improper dosage,irrational drug combination and some other irrational phenomenon after intervention were significantly lower than those of before intervention,with statistically significant difference（ P〈0. 05）. CONCLUSIONS： The intervention on irrational application of glucocorticoids can further standardize the clinical application of glucocorticoids.
