舒筋活血汤配合渐进式功能锻炼对踝关节骨折术后功能恢复的影响

Clinical study of Shujin-Huoxue decoction combined with gradually functional exercise for the patients with ankle fracture postoperative recovery

目的探讨舒筋活血汤配合渐进式功能锻炼对踝关节骨折术后功能恢复的影响。方法将符合入选标准的60例踝关节骨折术后患者采用随机数字表法分为2组，每组30例。观察组在踝关节骨折术后采用舒筋活血汤内服与中药外敷结合渐进式功能锻炼治疗；对照组在踝关节骨折术后联合外用复方七叶皂苷钠凝胶并配合渐进式功能锻炼。2组均治疗2个月。采用Baird．Jackson量表评价踝关节功能，用量角器测量踝关节背伸角度、跖屈角度的主动活动度，评价临床疗效。结果观察组优良率为90．0％（27／30）、对照组为73．3％（22／30），2组比较差异有统计学意义（Х^2=2．004，P=0．039）。治疗后，观察组背伸角度[（15．69±3．02）。比（13．50±2．36）。，t=6-314]和跖屈角度[（32．07±0．34）。比（27．95±5．40）°t=10．205]高于对照组（P〈0．05）。结论舒筋活血汤配合渐进式功能锻炼可改善踝关节主动活动度，提高临床疗效。

Objectives In order to discuss the effect of Shufin-Huoxue decoction combined with gradually functional exercise for the ankle fracture postoperative function recovery. Methods A total of 60 patients with ankle joint fracture were treated by random number table method, and were divided into the observation and control groups, 30 patients in each group. The observation group was treated with the Shujin-Huoxue decoction orally and topical application and gradually functional exercise therapy; while the control group was treated with compound sodium aescinate gel for topital application combined with gradual functional exercise therapy. Two months were a course, so both groups were treated a course. The stretching and flexion angles were measured by protractor. The ankle joint function was measued by the Baird-Jackson rating scale. The clinical curative effect was evaluated. Results The total effect rate in the observation group was 90.0% （27/30） and control group was 73.3% （22/30）. There was significant difference between two groups （2,2=2.004, P=-0.039）. After treatment, the feet stretching Angle （15.69°± 3.02° vs. 13.50° ± 2.36°, t=-6.314） and plantar flexion Angle （32.07° ±0.34° vs. 27.95°± 5.40°, t=10.205） in the observation group were significantly higher than those in the control group （P〈0.05）. Conelusions The Shujin-Huoxue decoction combined with gradually functional exercise could improve the ankle activity and the clinical curative effect.
