独活寄生汤联合刺络放血法治疗老年膝关节炎临床疗效及安全性研究

Study on Effect and Safety of Duhuo Jisheng Decoction Combined with Bloodletting in Treatment of Senile Knee Arthritis in Gerontism

目的:探讨独活寄生汤联合刺络放血法治疗老年膝关节炎的临床疗效及安全性。方法:研究对象病例均为本院诊断及治疗的老年膝关节炎患者112例,按随机数字表法分组,对照组56例予以西医治疗;研究组56例予以独活寄生汤联合刺络放血法治疗,评定WOMAC疼痛量表评分,检测血清及关节液细胞因子变化,同时对比临床疗效及并发症。结果:对照组临床有效率76.79%与研究组临床有效率91.07%数据对比,有差异(P<0.05);相对于治疗前,两组治疗后疼痛评分、关节僵硬评分、生理功能评分及总分均显著降低,治疗后血清及关节液IL-1、TNF-α水平均降低,关节液CTX-Ⅱ水平均升高,有差异(P<0.05);相对于对照组,研究组治疗后疼痛评分、关节僵硬评分、生理功能评分较低,治疗后血清及关节液IL-1、TNF-α较低,关节液CTX-Ⅱ较高,有差异(P<0.05)。结论:独活寄生汤联合刺络放血法治疗老年膝关节炎疗效显著,能降低炎症指标,提高膝关节活动功能。

Objective：To explore the effect and safety of Duhuo Jisheng Decoction combined with bloodletting in the treatment of senile knee arthritis. Methods：A total of 112 gerontism cases with senile knee arthritis were selected and divided into 2 groups according to the random number table. The control group received conventional therapy and the treatment group additionally re- ceived Duhuo Jisheng Decoction combined with bloodletting, and WOMAC pain scale score, serum and joint fluid cell factor, clini- cal efficacy and complications were compared after treatment. Results ：The effective rate in the control group（76.79% ） was lower than that of the treatment group （91.07%） （ P 〈 0.05 ）. Compared with the control group, pain scores and joint stiffness score, physiological function and the total score of decreased after treatment in the study group. After treatment, the serum and synovial fluid of IL - 1 and TNF - ct levels were lower, synovial fluid CTX II levels were higher, with statistical significance （ P 〈 0.05 ）. Conclusion ： Duhuo Jisheng Decoction combined with pricking bloodletting in treatment of elderly knee osteoarthritis curative effect significantly, and can reduce markers of inflammation and improve the function of knee joint.
