宫血宁对剖宫产后放置宫内节育器产妇血清炎性指标、出血情况的影响

Effect of Gongxuening on serum inflammatory index and bleeding for women placing the IUD after cesarean section

目的 观察宫血宁对于剖宫产后放置宫内节育器产妇血清炎性指标、出血情况的影响,分析宫血宁在剖宫产后宫内节育器放置中的作用价值。方法 选取浙江省绍兴市嵊州市妇幼保健院于2014年3月~2016年3月期间剖宫产术后自愿接受宫内节育器放置的156例妇女作为观察对象,根据术后是否应用宫血宁分为2组,即宫血宁组75例（使用宫血宁＋诺氟沙星）和对照组（使用诺氟沙星）81例,对比分析2组手术前后血清炎性指标的变化,同时对比分析2组子宫出血情况。结果 手术后,2组肿瘤坏死因子-α（tumor necrosis factor-α,TNF-α）、白细胞介素-6（interleukin-6,IL-6）、白细胞介素-10（interleukin-10,IL-10）均出现明显波动,且同时间点宫血宁组TNF-α、IL-6、IL-10的术后24h、术后7d均显著低于对照组,差异有统计学意义（P〈0.05）;术后宫血宁组WBC术后24h、术后7d显著低于对照组宁组,而Hb术后24h、术后7d显著高于对照组,差异有统计学意义（P〈0.05）;宫血宁组产妇出血量2h、24h、出血停止时间、产褥持续时间显著低于对照组,差异有统计学意义（P〈0.05）。结论 剖宫产后放置宫内节育器产妇使用宫血宁胶囊治疗,不仅能够有效抑制血清炎性指标表达,而且有利于缩短子宫出血时间、减少出血量、防止术后贫血,建议临床多加重视。

Objective To observe the effect of Gongxuening for the women placing the IUDafter cesarean section, and to analyze the effect and value of Gongxuening on intrauterine device placement after cesarean section. Methods 156 women who received voluntary intrauterine device after cesarean sectionin theShengzhou Maternal and Child Health Hospital of Shaoxing, Zhejiang were as the observation object. According to whether the application of Gongxuening, they were divided into two groupsafler treatment, the Gongxuening group of 75 cases （using Gongxuening ＋ Nifloxacin） and the control group of 81 cases （using norfloxacin）. The changes of serum inflammatoiy indexes were compared between the two groups before and after operation, and the uterine bleeding was compared between the two groups. Results After operation, the levels o f tumor necrosis fector-a （TNF-a）, interleukin-6 （IL-6） and interleukin-10 （IL-10） were with a significantfluctuation, and at the same time, Gongxuening group TNF-a, IL-6, IL-10,24hours after suigery, postoperative 7d was significantly lower than the control group, the difference was statistically significant （P〈 0.05）. After treatment, the WBC of Gongxuening group 24hours after surgery, postoperative 7d was significantly lower than the control group, but flie Hb 24 hours after surgey was significantly higher than the control group, the difference was statistically significant （P〈0.05）. The bleeding volume2h, 24h, bleeding stop time and puerperium duration in the Gongxuening groiq） were significantlylower than the control group, the difference was statistically significant （P〈0.05）. Conclusion After cesarean section maternal IUD use Gongxuening capsule treatment, not only can effectively inhibit the expression of serum inflammatory markers, and can shorten the time of uterine bleeding, reduce bleeding, prevent postoperative anemia. It is suggested to pay more attention to clinical treatment.
