隐神经大隐静脉营养血管皮瓣修复小腿及足跟部软组织缺损 被引量:8

摘要 目的探讨隐神经大隐静脉营养血管皮瓣修复小腿及足跟部软组织缺损的临床效果。方法采用隐神经大隐静脉营养血管皮瓣修复16例小腿及足跟部软组织缺损患者。受区创面面积4 cm×5 cm^6 cm×16cm,供区皮瓣面积5 cm×7 cm^8 cm×18 cm,供区一期游离皮片移植修复。结果患者均获得随访,时间6个月~3年。14例皮瓣完全成活;2例皮瓣远端约1 cm宽皮肤坏死,基底部有软组织覆盖,经换药后愈合。1例皮瓣臃肿行皮瓣修整术。未发生血管危象,骨折无骨不连发生。供区皮片移植均一期成活,无明显功能障碍。结论隐神经大隐静脉营养血管皮瓣修复小腿及足跟部软组织缺损效果良好。 Objective To investigate the clinical effect of saphenous nerve great saphenous vein fasciocutaneous flap for repair of leg and heel soft tissue defect. Methods Saphenous neuro vascular flaps were used to repair the defect of parenteral nutrition in 16 patients with leg and heel soft tissue. The affected area was 4 cm × 5 cm - 6 cm × 16 cm,the donor site skin flap area was 5 cm × 7 cm - 8 cm × 18 cm,and the donor site was a free skin graft repair. Results All patients were followed up for 6 months - 3 years. 14 cases of flaps survived completely; 2 cases had skin necrosis( about 1 cm wide) at the distal end of the skin flap,and the basal part had soft tissue coverage and healed after dressing change. One case flap bloated had to be repaired. There was no vascular crisis,without fracture nonunion.There was no obvious functional disorder in the homogeneous stage of skin graft transplantation in donor site. Conclusions It is good that the saphenous nerve great saphenous vein fasciocutaneous flap is used to repair soft tissue defect of the leg and heel effect.
