头面颈穴位按摩联合冰刺激治疗脑卒中后吞咽障碍疗效观察

The therapeutic effect of acupoint massage in head face and neck combined with ice stimulation for stroke swallowing dysfunction

目的:评价中医手法联合冰刺激为主的治疗对脑卒中后吞咽障碍的临床疗效。方法:选取脑卒中吞咽障碍康复治疗的110例,随机分为观察组和对照组,各55例。对照组进行低频电刺激、吞咽肌功能锻炼及常规药物治疗;观察组在此基础上加用穴位按摩(风池、风府、哑门、颊车、下关、承浆、廉泉及夹廉泉穴)及冰刺激治疗。观察治疗前后洼田饮水实验情况。结果:两组治疗后吞咽障碍均有所改善(P<0.01)。观察组痊愈8例,显效12例,28例治疗有效,7例治疗无效,总有效率为87.2%。对照组3例痊愈,6例显效,有效34例,12例治疗无效,总有效率为78.1%。结论:穴位按摩及冰刺激的综合治疗对恢复脑卒中后吞咽障碍患者的吞咽能力具有积极意义。

Objective ：To evaluate the therapeutic effect of acupoint massage in head ,face ,and neck in com-bined with ice stimulation for stroke swallowing dysfunction in clinical .Methods ：110 patients with stroke swallo-wing dysfunction were enrolled in this study and divided in the observation group and the control group （55 patients for each group） .The patients in the control group were treated with low -frequency pulsed electrical stimulation , medicine and rehabilitative training .The patients in the observation group were additionally treated with acupoint massage in head ,face ,and neck （Fengchi ,Fengfu ,Yamen ,Jiache ,Xiaguan ,Chengjiang ,Lianquan and Jialianquan） in combined with ice stimulation .The difference between the two groups and the therapeutic effect were evaluated by the water-swallowing test .Results ：The sallowing function of patients in the two groups was all significantly im-proved after respective treatment （ P〈0 .01 ） .The combined treatment for the observation group cured 8 cases , markedly improved 12 cases ,improved 28 cases and was invalid in 7 cases ,and the effective rate was 87 .2% .The treatment for the control group cured 3 cases ,markedly improved 6 cases ,improved 34 cases and was invalid in 12 cases ,and the effective rate was 78 .1% .Conclusion ：Compared with merely applying low-frequency pulsed electrical stimulation ,medicine and rehabilitative training ,the acupoint massage in head ,face and neck in combined with ice stimulation can markedly improve the swallowing function in stroke patients with swallowing dysfunction .
