蒋士卿教授运用阳和汤加蜈蚣治疗原发性恶性骨肿瘤经验

Experience of Professor Jiang Shiqing Treatment of Primary Malignant Bone Tumor with Yang He Decoction and Centipede

蒋士卿教授提出,原发性恶性骨肿瘤形成的根本病机是肾阳不足,导致寒凝、痰湿、瘀血等阴邪凝积于骨而成,以肾阳虚为本,以寒凝、痰湿、瘀血为标;痰瘀互结是其发病的重要病机;风邪、痰毒、瘀血是发生骨肿瘤转移的关键因素。治疗上,蒋士卿教授辨病选方,异病同治,以温阳通滞,活瘀散结为基本治疗大法,选用阳和汤加蜈蚣作为基础方治疗,处方为:熟地黄30 g,鹿角胶(烊化)30 g,炮姜15 g,肉桂(后下)10 g,麻黄10 g,炒白芥子20 g,生甘草10 g,大蜈蚣粉(冲服)3g。同时辨证施治,随症加减,并且主张中西医结合,取长补短,以提高治疗效果,但应避免西医的过度治疗。

Professor Jiang Shiqing holds that the formation of primary malignant bone tumor is Kidney Yang deficiency, leading to cold coagulation, phlegm dampness and blood stasis in bone. Phlegm and blood stasis is the important mechanism in development of primary malignant bone tumor. Kidney Yang deficiency is the basic pathogenesis of this cancer, and cold coagulation, phlegm damp- heSS and blood stasis are the appearance. Pathogenic wind, sputum poison and blood stasis are the key factors in bone metastasis. So Professor Jiang Shiqing diagnoses diseases to choose prescription and treats different diseases the same prescription uses warming Yang and dredging blockage, and removing stasis as the basic therapeutic principle. He puts Yang He Decoction with centipede as the basic prescription for treatment. Prescription： Shudihuang （Prepared Radix Rehmanniae） 30 g, Lujiaojiao （Antler Glue） 30 g （melt）, Paojiang （ Roasted Ginger） 15 g, Rougui （Cinnamon） 15 g （later）, Mahuang （Ephedra） 10 g, Chaobaijiezi （ Roasted Sina- pis Seeds） 20 g, Shenggancao（ Licorice）10 g,Wugongfen（ Centipede Powder） 3 g（blunt）. At the same time, combining with syn- drome differentiation and treatment in clinical practice. And he also advocates that combining with traditional Chinese and western medicine to learn from each other, aiming to improve therapeutic effect, but avoid over treatment of western medicine.
