白虎汤联合还原型谷胱甘肽治疗重症肺炎伴高热临床研究

Clinical Study on Effect of Baihu Decoction Combined with Reduced Glutathione in Treatment of Severe Pneumonia with High Fever

目的:观察白虎汤联合还原型谷胱甘肽治疗重症肺炎伴高热的临床研究。方法:76例重症肺炎伴高热患者按随机数字表法平均分为对照组和观察组。对照组38例给予还原性谷胱甘肽治疗,观察组38例在对照组治疗基础上给予白虎汤。比较两组患者临床疗效、肺功能、临床客观指标、炎症因子及不良反应。结果:治疗后,对照组有效率为73.68%,显著低于观察组有效率(92.11%),差异具有统计学意义(P<0.05);与对照组比较,观察组发热消失时间、三凹征消失时间、气促消失时间、肺部啰音消失时间及住院时间均缩短(P<0.05);治疗后两组患者白细胞、血小板及中性粒细胞均降低(P<0.05),观察组以上指标降低更显著(P<0.05);治疗后血清肿瘤坏死因子-α、超敏C反应蛋白含量降低(P<0.05),观察组以上指标降低更显著(P<0.05);治疗后两组第1秒用力呼气量、用力肺活量、呼气峰流速及最大吸气压、最大呼气压均升高(P<0.05),观察组以上指标升高更显著(P<0.05)。结论:白虎汤联合还原型谷胱甘肽治疗重症肺炎伴高热效果显著,可显著缓解炎症反应,恢复肺功能。

Abstract： Objective：To investigate the clinical effect of Baihu Decoction combined with reduced glutathione in the treatment of se- vere pneumonia with high fever. Methods：76 cases with severe pneumonia with high fever were selected and divided into two groups randomly. 38 cases in the control group were treated with reduced glutathione, and 38 cases in the observation group re- ceived more with Baihu Tang. Clinical remission status, clinical objective indexes,inflammatory factors and pulmonary function in- dexes, clinical curative effect and adverse reaction conditions were compared after treatment. Results：The clinical efficiency in the control group （73.68 % ） was lower than the observation group （ 92.11% ） , with significant difference （ P 〈 0.05 ） ; compared with the control group, the fever disappeared time, three concave syndrome disappeared time, shortness of breath, pulmonary rales disap- peared time and hospitalization time disappear of the observation group were shorter（ P 〈 0.05 ）. After treatment, two groups of pa- tients with white blood cells, platelets and neutrophils were decreased （ P 〈 0.05 ）, the observation group was significantly lower in the above indicators （P 〈 0.05）. After treatment, serum tumor necrosis factor-alpha, high sensitivity C reactive protein content de- creased （ P 〈 0.05 ）, the observation group was significantly lower （ P 〈 0.05 ）. After treatment, two groups of forced expiratory volume in the first second, forced vital capacity, peak expiratory flow rate and maximum inspiratory pressure, the maximum expira- tory pressure were increased （ P 〈 0.05 ） , the observation group increased significantly in the above index （ P 〈 0.05 ）. Conelusion ： Baihu Decoction combined with reduced glutathione in the treatment of severe pneumonia with high fever has significant effect and it can relieve the inflammatory response and restore the lung function.
