云南白药治疗会阴切口愈合不良产妇的临床观察

Clinical Observation on Yunnan Baiyao Powder on Episiotomy Wound Healing

目的观察分析会阴切开缝合术产妇术后外用云南白药对切口愈合不良的影响。方法选取单胎首次分娩自然分娩并采用会阴切开缝合术治疗的产妇200例,随机分为两组。对照组在术后给予常规碘伏消毒处理,治疗组给予云南白药粉剂以生理盐水调制糊状后外涂创口表面。观察比较两组患者愈合程度、切口愈合不良发生情况、愈合时间及疼痛程度差异。结果治疗组患者甲级愈合率明显高于对照组(P<0.05)。对照组切口线头周围化脓、切口硬结、切口裂开不愈发生率高于治疗组(P<0.05)。治疗组会阴肿胀发生率低于对照组(P<0.05)。治疗组疼痛VAS评分低于对照组,愈合时间短于对照组(均P<0.05)。结论会阴切开缝合术后外用云南白药能够有效的提高切口甲级愈合率,缩短愈合时间,减少切口愈合不良情况的发生,降低患者的疼痛程度,值得临床推广应用。

Objective To observe and analyze the external effects of Yunnan Baiyao powder on wound healing after episiotomy. Methods 200 cases of natural childbirth delivery primipara who received episiotomy and synthetic treatment were randomly divided into treatment group and control group,100 cases in each group. Control group was given routine iodophor disinfection after operation,observation group was treated with Yunnan Baiyao powder paste with saline water coated on wound surface. To observe and compare the healing of wound,adverse reactions,healing time and pain degrees between the two groups. Results The Class-A healing rate of treatment group was 99. 0%,higher than91. 0% of control group with statistical significant difference（ P〈0. 05）. The incision thread around fester,wound induration,wound dehiscence healing rate of control group were higher than those of treatment group with statistically significant difference（ P〈0. 05）. The perineal swelling incidence of treatment group was 3. 0%,higher than 11. 0% of control group with statistically significant difference（ P〈0. 05）. The pain VAS score of treatment group was lower than that of control group,the healing time was shorter than that of control group with statistically significant difference（ P〈0. 05）. Conclusion Yunnan Baiyao powder is effective in treatment of episiotomy wound healing,it can effectively improve Class-A healing rate,shorten the healing time,reduce the poor wound healing situation and reduce the pain degree of patients,it is worthy of clinical application.
