老年复杂性肱骨近端骨折钢板内固定术后电磁疗法配合中药烫疗干预临床研究 被引量:5

摘要 目的:观察老年复杂性肱骨近端骨折钢板内固定术后电磁疗法配合中药烫疗干预临床研究。方法:134例随机分为两组各67例,两组均采用钢板内固定术治疗,术后采用常规抗感染及康复治疗,观察组联合电磁疗法及中药烫疗。结果:两组肩关节活动范围、疼痛、功能及解剖评分均升高,但观察组更显著(P<0.05);观察组骨性愈合时间,平均可负重时间较对照组更短,优良率更高(P<0.05);对照组发生肱骨头坏死7例、肱骨头轻度内翻4例、切口感染3例、总发生率20.89%,观察组则切口感染2例、总发生率2.99%,两组并发症发生率比较差异有统计学意义(P<0.05)。结论:电磁疗法配合中药烫疗可促进老年复杂性肱骨近端骨折钢板内固定术后病情康复,使并发症发生率降低。 Objective : To study the intervention of electromagnetotherapy combined with hot medicated compress with Chinese herbs on elderly patients with complex proximal humeral fracture after the operation of internal fixation with steel plate. Method: 134 cases were divided into 2 groups evenly in random, 67 cases in each group. Both groups were treated with internal fixation with steel plate and were given anti-infective and rehabilitation therapy after the operation and the observation group were treated with electromagnetotherapy combined with hot medicated compress with Chinese herbs additionally. Result: The range of motion ( ROM ) , pain, function and anatomical score of the shoulder joint in both groups increased, but that of the observationgroup increased more ( P〈0.05 ) . The bone healing time and the average weight bearing time of the observation group was shorter than that of the control group and the total excellent rate of the observation group was higher than that of the control group ( P〈0.05 ) .There was 7 cases of humeral head osteonecrosis, 4 cases of humeral head varus deformity and 3 cases of incision infection, with the overall incidence of 20.89% in the control group while there was 2 cases of incision infection, with the overall incidence of 2.99% in the control group. There was statistical significance in the complication rate between the two groups ( P〈0.05 ) . Conclusion: Electromagnetotherapy combined with hot medicated compress with Chinese herbs could promote rehabilitation of elderly patients with complex proximal humeral fracture after the operation of internal fixation with steel plate and reduce the complication rate.
