加味独活寄生汤联合关节镜下清理术治疗膝骨关节炎54例临床研究

Clinical Study of Using Modified Duhuo Jisheng Decoction Combined with Arthroscopic Debridement in Treating 54 Cases Knee Osteoarthritis

目的:观察加味独活寄生汤联合关节镜下清理术治疗肝肾亏虚型膝骨关节炎(KOA)的临床疗效。方法:将54例肝肾亏虚型KOA患者随机分为对照组(27例)和治疗组(27例)两组,观察两组患者治疗前后膝关节疼痛程度和膝关节功能的变化。结果:经治疗后,对照组总有效率为70.4%(治愈4例,显效7例,有效8例,无效8例);治疗组总有效率为92.6%(治愈9例,显效9例,有效7例,无效2例)。治疗组疗效显著优于对照组(P<0.05);治疗组治疗后患者的膝关节疼痛程度显著降低,膝关节功能明显提高,且效果明显优于对照组(P<0.05)。结论:加味独活寄生汤联合关节镜下清理术可有效延缓膝骨关节炎疾病进程,促进关节软骨修复,显著改善肝肾亏虚型KOA患者的临床症状,明显提高疗效,值得临床推广运用。

Objective： To observe the clinical effect of using Modified Duhuo Jisheng Decoction combined with arthroscopic debridement in the treatment of liver and kidney deficiency type of knee osteoarthritis （KOA）. Methods ： 54 cases of type of defi- ciency of liver and kidney KOA patients were randomly assigned for controlled group （27 cases） and treatment group （27 cases）, to observe changes of knee pain and knee function before and after the treatment, the patients in the two groups. Results： After treatment, the total effective rate of the controlled group was 70. 4% （4 cases were cured, 7 cases were markedly effective, 8 cases were effective, 8 cases ineffective）. The total effective rate was 92. 6% in the treatment group （9 cases were cured, 9 ca- ses were markedly effective, 7 eases were effective, 2 cases were ineffective）. The curative effect of the treatment group was sig- nificantly better than the controlled group （P 〈 0. 05） ; the knee pain of the treatment group was significantly lower, the knee function was significantly improved, and the effect was significantly better than the controlled group （P 〈 0. 05 ）. Conclusion： Modified Duhuo Jisheng Decoction combined with arthroseopy cleaning operation can be effective in delaying the progression of knee osteoarthritis disease and promote the repair of articular cartilage and significantly improve the clinical symptoms of type of deficiency of liver and kidney of patients with KOA, significantly improve the curative effect, worthy of clinical application.
