独活寄生汤加味方热熨配合关节冲洗治疗膝关节滑膜炎临床研究

Clinical study of Using Duhuo Jisheng Decoction Hot Compress Combined with Joint Irrigation in the Treatment of Gonarthromeningitis

目的:观察独活寄生汤加味方热熨配合关节冲洗治疗膝关节滑膜炎的疗效。方法:将120例膝关节滑膜炎患者随机平均分为三组,分别采用独活寄生汤加味方热熨配合关节冲洗、单纯的独活寄生汤加味方热熨及单纯的关节冲洗疗法治疗,分别于治疗后第7天、第14天对患者膝关节的疼痛、活动度及肿胀度进行评价。结果:治疗7天、14天后,三组患者膝关节疼痛、活动度及肿胀情况均较治疗前好转(P<0.05);观察组疼痛、肿胀效果优于对照1、2组(P<0.05),观察组膝关节活动度优于对照2组(P<0.05),与对照1组疗效相近(P>0.05);对照1、2组间症状、体征无明显差异(P>0.05)。观察组有效率为95%,对照1组有效率为82.5%,对照2组有效率为80%。结论:独活寄生汤加味方热熨配合关节冲洗治疗膝关节滑膜炎疗效显著,在止痛、消肿、改善关节活动度等方面的有效率明显高于单纯的独活寄生汤加味方热熨及单纯的关节冲洗疗法,是膝关节滑膜炎的理想治疗方法,值得临床推广。

Objective： To observe the effect of Duhuo Jisheng Decoction hot compress combined with joint irrigation in the treatment of gonarthromeningitis. Methods ： 120 cases of knee joint sinusitis patients were randomly divided into three groups, re- spectively, using Duhuo Jisheng Decoction hot compress combined with joint irrigation, pure Duhuo Jisheng Decoction hot ironing and simple joint washing therapy, respectively on the 7th day and the 14th day after treatment of patients with knee joint pain, ac- tivity and swelling degree evaluation. Results ： In the treatment of 7 days, 14 days, the three groups of patients with knee joint pain, activity and swelling were compared with those before treatment improved （P 〈 0. 05 ）. In the observation group, the pain, swelling effect was better than that of the controlled group 1 and group 2 （P 〈 0. 05 ）. In the observation group the activi- ties of knee joint was better than that in controlled group 2 （ P 〈 0. 05 ）, and according to one group efficacy was similar （P 〉 0. 05） ; the controlled group 1 and between the two groups of symptoms and signs were without significant difference （P 〉 0. 05 ）. The effective rate of the observation group was 95%, and the effective rate of the controlled group was 82. 5%, and the con- trolled group 2 was 80%. Conclusion： Duhuo Jisheng Decoction hot compress combined with joint irrigation treatment knee joint synovitis curative effect is distinct, the efficiency in pain, swelling, improve joint mobility are significantly higher than that of pure Duhuo Jisheng Decoction hot compress and pure joint irrigation therapy. It is the ideal treatment for knee joint synovitis, worthy of clinical application.
