痛风性关节炎患者外周血白细胞中Toll样受体4表达与基因多态性研究

A Study on Toll Like Receptor 4 in Peripheral Blood Leukocytes Expression and Gene Polymorphism in Patients with Gouty Arthritis

目的对痛风性关节炎患者外周血白细胞中Toll样受体4(TLR4)表达与基因多态性予以分析,为痛风性关节炎的早期诊断和治疗提供参考。方法选择2015年1月—2016年6月右江民族医学院附属医院确诊为痛风性关节炎患者及同期体检健康者为观察组和对照组,每组各80例。均行外周血白细胞中TLR4表达测定和基因型鉴定并进行比较和评价。结果观察组外周血白细胞中TLR4 mRNA、核因子-κBD65、血浆IL-1β及血尿酸水平均显著高于对照组(P<0.05);观察组GGb、GT及G等位基因b与对照组比较差异无统计学意义(P>0.05),观察组TT和T等位基因均显著高于对照组(P<0.05)。结论痛风性关节炎患者外周血白细胞中TLR4表达与TT和T等位基因均表现出较强的表达及较高的的比率,对临床诊断及治疗具有一定的指导价值。

Objective To analyze expression of Toll like receptor 4（ TLR4） in peripheral blood leukocytes and gene polymorphism of patients with gouty arthritis in order to provide reference for early diagnosis and treatment. Methods Patients with gouty arthritis during January 2015 and June 2016 were selected as observation group（ n = 80）,and healthy people taking medical examination at the same period were selected as control group（ n = 80）. TLR4 expressions in peripheral blood leukocytes and gene identification were detected,and results were compared and evaluated. Results Compared with those in control group,in observation group,TLR4 mRNA,nuclear factor-k BD65,plasma interleukin-1（ IL-1） and serum uric acid levels were significantly higher（ P〈0. 05）; there were no significant differences in GGb,GT and G allelesb（ P〉0. 05）; and TT and T alleles were significantly higher（ P〈0. 05）. Conclusion Expression of TLR4 in peripheral blood leukocytes and rates of TT and T alleles in patients with gouty arthritis are significantly higher,and therefore it has a certain guiding value for clinical diagnosis and treatment.
