糖化血红蛋白在妊娠期糖尿病筛查中的应用 被引量:2

摘要 目的探讨糖化血红蛋白(HbA1c)对妊娠期糖尿病(GDM)早期筛查的临床应用价值。方法选取505例产前检查的孕妇,均给予HbA1c和口服75 g葡萄糖(OGTT)试验。根据2010年国际妊娠与糖尿病研究组织制定的GDM新诊断标准,其中55例诊断为GDM,450例为正常孕妇。结果 GDM组孕妇HbA1c和OGTT结果均显著高于正常孕妇组差异具有统计学意义(P<0.05);HbA1c阳性检出率显著高于OGTT,两组比较,差异具有统计学意义(P<0.01)。结论HbA1c和OGTT在妊娠期糖尿病的筛查中,具有同样高的应用价值,且前者更易操作、诊断符合率高和影响因素少等特点,在临床实践中值得推广。 Objective To investigate the clinical value of HbA1 c in the screening of gestational giabetes mellitus. Methods 505 pregnant women took HbAlc and 75 g oral glucose test (OGTT). According to new diagnostic criteria of the international pregnancy and diabetes research organization in 2010, 55 cases were diagnosed with GDM, and 450 cases were diagnosed as normal pregnant women. The statistical analysis of the data was based on the SPSS software. Results Compared with the healthy pregnant women, HbA1 c and OGTT were significantly higher in the GDM group. The difference was statistically significant (P〈0.05). The positive detection rate of HbA1 e was significantly higher than that of OGTT. The difference was statistically significant (P〈0. 01 ). Conclusion In the screening of gestational diabetes, both HbAlc and OGTT have high value, and the former one has such merits as easier operation, higher diagnostic accuracy and fewer influence factors. It is worth popularizing in clinical practice.
